摘　　要：目的　探讨基质金属蛋白酶9(MMP 9)在胎膜早破发病机制中的作用。方法　选择足月胎膜早破(未临产)产妇32例和早产胎膜早破(未临产)产妇11例作为研究组,11例足月胎膜完整(未临产)产妇作为对照组;采用免疫组织化学SP法检测产妇胎膜宫颈部和宫体部中MMP 9的分布与表达。结果　MMP 9在研究组产妇胎膜羊膜上皮细胞和绒毛膜细胞中均可见表达,在对照组胎膜中未见表达;研究组MMP 9 在宫颈部胎膜的表达高于对照组(P<0.01);研究组MMP 9 在宫体部胎膜的表达高于对照组(P<0.01);研究组MMP 9在宫颈部胎膜的表达高于在宫体部的表达(P<0.01)。结论　宫颈部胎膜MMP 9的高表达是导致该处胎膜发生破裂的重要原因之一。Objective To explore their role in the pathogenesis of PROM.Methods FM samples(cervical membranes and membranes in corpus of uterus) were obtained from the following three groups: ① women(n=11) not in labor with PPROM, ② women(n=32) not in labor with PROM at term, ③ term pregnants(n=11) not in labor with intact membranes(the controls). MMP -9 was assessed by S -P immunohistochemistry.Results ①We had only detected MMP -9 in amniotic epithelial cells and chorionic cells of study groups; ②IH scores of MMP -9 in cervical membranes was obviously higher in study groups than the controls(P< 0.01 ); ③IH scores of MMP -9 in corpus membrane showed significant deference between study groups and the controls(P< 0.01 ); ④Expression of MMP -9 in cervical membranes of study groups was significant higher than that in corpus membranes (P< 0.01 ).Conclusion The higher expression of MMP -9 in cervical membranes contribute to the rupture in the local area of PROM.
