摘 要:目的探索潮州某驻军登革热和流行性乙型脑炎(乙脑)等虫媒病综合防制效果,为预防和控制虫媒病毒病提供科学依据。方法收集发热可疑患者和健康人血标本及蚊虫标本作病毒分离和血清学抗体调查;按全面或随机法调查营区内蚊虫孳生情况,人诱法监测成蚊叮刺率;用C6/36细胞培养法分离病毒并用间接免疫荧光法(IFAT)或逆转录聚合酶链反应(RT PCR)法鉴定;用IFAT调查健康人群登革热和乙脑抗体;以“治本清源”为原则,普及防病知识和开展爱国卫生运动,针对不同孳生地类型和栖息特点采用不同方法处理。结果经监测发热待查病人和媒介蚊虫均未分离出病毒;健康人群登革热和乙脑的抗体水平不高;处理前蚊类优势种为白纹伊蚊和致倦库蚊,有少量三带喙库蚊和骚扰阿蚊,其中白纹伊蚊的容器指数(CI)为24.1,百户指数(BI)为30.0,均高于规定的危险地区界限20.0。结论该驻地存在登革热和乙脑暴发流行潜在危险,蚊虫密度较高,人群抗体水平低,采用综合防制措施后,经3年连续观察,蚊媒已控制在不足为害的水平。Objective To carry out the epidemiological surveillance of dengue fever(DF) and Japanese B encephalitis(JBE), and provide effective integrated control strategy in army region of Chaozhou. The result may provide guidance for arbovirus control. Methods Investigation on breeding sites and population density of adult mosquitoes was made by random and systematic sampling methods in army region of Chaozhou where DF once happened in March-December from 1997 to 2000. ~C6/36 cell isolation combined with RT-PCR were used to detect virus of DEN and JBE in the sera of unexplained feverish patients and the mosquitoes pools. The IFAT was used to detect antibody of the healthy mass for DEN and JBE. The integrated control measures was made, including to inculcate knowledge about preventing disease and to apply different control methods on different kinds of breeding ground and habitats. Results The virus of DEN and JBE were not found through ~C6/36 cell isolation from both the sera of unexplained feverish patients and the mosquito pools. As for seroepidemiology, antibody level of the healthy mass for DEN and JBE was not high on the whole (~≤4.4% ) and dengue antibody level was decreasing with time. But, as for vector mosquito, there were four species in the investigated region. Aedes albopictus and Culex pipiens pallens were the dominant species. The Aedes albopictus container index(CI) was 24.1 and the breteau index(BI) 30 before treatment, Which was highly above the effective control limit which was 5. Conclusion The epidemiological surveillance indicated that there was vector mosquito of DEN and JBE and Aedes albopictus density was higher in army distrct of Chaozhou. Controlling mosquitoes and keep the environment clean should be an effective method to prevent DEN and JBE in the region.
