摘  要:目的探讨血清心肌肌钙蛋白Ⅰ(CTn-I)在肺炎支原体肺炎(MPP)患儿心肌损伤早期的诊断价值。方法用化学发光法和全自动生化分析仪测定健康对照组(A组)、MPP组(B组)及MPP并疑似心肌损伤组(C组)各32例患儿血清心肌肌钙蛋白(CTn-I)水平及肌酸激酶心型同工酶(CK-MB)活性。分别测定各组CTn-I及CK-MB的阳性率,对结果进行统计学分析。结果健康对照组及MPP组CTn-I值均在正常范围,两组相比,P值>0.05,差异无显著性。MPP合并心肌损伤组与对照组CTn-I及CK-MB值比较,P值均<0.01,差异显著。对照组CTn-I及CK-MB值无1例阳性。MPP组CK-MB阳性率25%,CTn-I阳性率0%。MPP并心肌损伤组CK-MB及CTn-I阳性率分别为84%及97%。结论CTn-I的检测对诊断心肌损害具有高度灵敏性及特异性,在诊断支原体肺炎早期心肌损害中具有较高临床价值。Objective To observe the application value of cardiac troponin I(CTn-I)for the early diagnosis of myocardial damage of children with myoplasma phenumoniae phenumonia(MPP).Methods Serum CTn-I level and creatine Kinase isoenzyme MB(CK-MB)were respectively measured by automatic chemoluminescence and chemical assay in healthy control group (group A),MPP group without cardiovascular complication (group B),and MPP group with suspected cardiovascular complication(group C),they each grouped 32 cases.Results Concentrations of CTn-I and CK-MB in both group A and group B were normal.In comparison with the Data between,there was no difference(P>0.05),but concentrations in group C were significantly higher than group A or group B(P<0.01).Neither CTn-I nor CK-MB was positive in group A.The positive rate of CK-MB in group B was 25%,but the positive rate of CTn-I in group B was 0%,The positive rate of CK-MB and CTn-I was separately 84% and 92%.Conclusions CTn-I as one of the markers of myocardial damage is more sensitive and specific than CK-MB.CTn-I has significant clinical value in early diagnosis of MPP patients with myocardial damage.
