摘 要:目的探讨Sysmex F-800 Microcell counter在区分肾小球性血尿与非肾小球性血尿中的应用价值。方法应用Sysmex F-800 Microcell counter对400例血尿病人进行了自身血和尿红细胞平均体积(MCV)及其比值测定,然后与临床及病理诊断进行对照,并将数据按两样本均数的t检验进行统计学处理。结果肾小球性血尿患者的血红细胞MCV在83-108 fl之间,尿红细胞MCV在40-78 fl之间,自身血和尿红细胞MCV比值为1.3- 2.1。非肾小球性血尿者血红细胞MCV在79-112 fl之间,尿红细胞MCV在76-136 fl之间,自身血和尿红细胞MCV比值为0.76-1.0。结论尿红细胞MCV<77 fl,血和尿红细胞MCV比值>1.3为肾小球性血尿,论断符合率为98.8%;尿红细胞MCV≥77 fl,血和尿红细胞MCV比值≤1为非肾小球性血尿,符合率为98.3%,测定血尿病人自身的血和尿红细胞MCV及其比值不受检验人员主观因素的影响,可以更客观、准确地区分血尿来源。Objective To explore the applicant value of Sysmex F-800 Microcell Counter in indentifying the source of hematuria of patients with glomerular or non-glomerular hematuria. Methods Four hundred patients with hematuria were examined the urine and peripheral venous-blood by Sysmex F-800 Microcell Counter, then analysis with the diagnosis of clinical and the pathological, the data were computed by statistics. Results The blood MCV of glomerular hematuria were 83 - 105 fl, urinary MCV were 40-78 fl, MCV ratio of biood to urine were 1.3-2.1. While non glomerular hematuria the blood MCV were 79- 112 fl, urinary MCV were 76- 136 fl.MCV ratio of blood to urine were 0.76- 1.0. Conclusion Urinary MCV < 77fl, MCV ratio of blood to urine > 1.3, they were glomerular hematuria, conformity rate was 98.8 % ; when urinary MCV ≥ 77 fl, MCV ratio of blood to urine <1, they were non-glomerular hematuriar, conformity rate was 98.3% . Determination MCV and their ratio of blood and urine of patients with hematuria can be used to differentiated objectively the glomerular or non-glomerular hematuria with accuracy.
