老年男性糖尿病患者184例血压控制水平与大血管并发症相关性研究及护理

摘　　要：目的探讨老年男性糖尿病患者血压控制水平与大血管并发症之间的相关性,并确定相应的护理措施。方法将184例老年男性糖尿病患者依据血压状况分为5组:正常血压组,血压控制理想组,控制良好组,控制不良组及未控制组。观测各组血压与血管并发症的关系。结果184例中有高血压139例(75.5%),高血压患者中血压控制理想者44例(31.7%),控制良好者28例(20.1%),控制不良者42例(28.2%),未控制者25例(20.0%)。共出现冠心病127例(69.0%),心肌梗塞17例(9.2%),下肢血管病变73例(39.7%)。心肌梗死、足背动脉搏动异常、下肢血管病变的发生率在高血压组明显高于正常血压组,并随年龄增高、高血压病程延长和血压水平增高而明显增加,均有统计学差异。结论老年男性糖尿病患者的血压水平影响其大血管并发症的发生,护士应明确其重要性,加强护理及宣教,督促患者及家属重视血压控制,以防治大血管并发症。Objective To study the relationship between blood pressure(BP) control and vascular complication in senile male diabetic patients,and propose appropriate nursing strategies.Methods 184 senile male diabetic patients were divided in 5 groups according to BP levels:normal group,perfectly controlled group, well controlled group, poorly controlled group and uncontrolled group. The differences in vascular complications among the 5 groups were studied.Results Of the 184 patients, 139 patients (75.5%).had hypertension. Of the 139 hypertensive patients, BP was perfectly controlled in 44 patient(31.7%),well controlled in 28 patients (20.1%), poorly controlled in 42 patients (28.2%), and uncontrolled in 25 patients (20.0%). There were 127 cases (69.0%) of coronary heart disease, 17 cases (9.2%) of myocardial infarction,and 73 cases (39.%) of vascular disease of the lower extremities. The statistical analysis indicated that with age increasing, the course of hypertension became longer,BP level increased, and the incidence of vascular complication increased significantly.Conclusion Blood pressure in senile male patients with diabetes influences the incidence of vascular complication. Nurses should be aware of its significance,promote health education,and arouse attention of patients and families to BP control so as to prevent vascular complication from occurring.
