社区糖尿病防治模式的初探

摘　　要：[目的]　探索社区糖尿病防治模式及二级医院的作用与地位。　[方法]　以科研为契机,以糖尿病流行病学调查为切入点,建立四位一体的网、线、点、面防治模式。　[结果]　上海市平凉社区糖尿病患病率为9. 9%,糖调节受损患病率为11. 99%,高血压患病率为35. 46%,代谢综合征患病率为13. 38%。建立了社区糖尿病防治队列人群,对糖尿病及高危人群进行分级管理与干预。社区卫生服务中心人员综合素质得到提高,社区人群对糖尿病防治的自觉性得到提高。　[结论]　在社区建立四位一体的糖尿病防治模式有较强的可行性、科学性和先进性。二级医院在其中有着重要的作用与地位。Objective] To explore the prevention and cure pattern of diabetes in community and the role and position of the grade two hospitals. [Methods] Epidemiological survey of diabetes was done in the community as a research topic. The prevention and cure pattern in four aspects, that was web, line, spot and in the plane was set up. [Results] The prevalence of diabetes, impaired glucose regulation, hypertension and metabolic syndrome were 9.9%, 11.99%, 35.46% and 13.38% respectively in Pingliang community Shanghai. The brigade for prevention and treatment diabetes was formed. The diabetes and the high-risk people have been managed and interfered in different levels. The comprehensive quality of people working in the center of community health service has been enhanced. The community people's level of consciousness to prevent and cure diabetes has been enhanced too. [Conclusion] It is practical, scientific and advanced to set the prevention and cure pattern in four aspects. Grade two hospitals play an important role and have an important positions here.
