超声心动图自动边缘检测评价左室容积的实验研究

摘　　要：超声心动图自动边缘检测技术（ＡＢＤ）是无创性心功能声学定量的新方法。为了评价该技术测量左室容积的准确性及操作方法对测量结果的影响．我们用塑料薄膜做了七个形状相似、大小不等的左室模型，并用ＡＢＤ技术测定其腔面积和容积．结果与实际容积作对照。发现ＡＢＤ测量的面积、容积与实际值高度用关（ｒ＝０．９０～０．９８．Ｐ＜０．０１～０．００１）。Ｓｉｍｐｓｏｎ法计算的容积与实际值的相关性较面积长度法更为密切，但结果者有不同程度的低估。不同方法圈感兴趣区会影响测量结果。Echocardiographic automated border detection (ABD)is a new way for non-invasive acoustic quantification (AQ) of heart function. In order to evaluate its validation and the operation influence upon result-s , we had made seven left ventricular models with similar shape and different dimension using plastic film.Eachmodel consisted of a cylinder and a conc. Its real volume is the liquid capacity it had. Models were put in a watergroove. Cavity areas and volumes in long axis views were obtained by HP Sonus 1500 AQ system. TheSimpson's rule and area-length formula were applied to calculate volumes respectively. The region of interest(ROI)was drawn in four ways . tracing along the inner edge of the model wall ; the outer edge of the rnodel wall .the midline between the inner and the outer edges of the model wall and 2mm away from the outer edge of themodel wall. Results : 1 . Volumes measured by ABDsystem correlated well with the real volume(r= 0. 90-0. 98.SEE==15. 92-26. 76ml P< 0. 01 0. 001) . The best correlation was obtained when the ROI was traced along the outer edge of the mudel wall(r=0.98 P<0.001)But the volumes measured by all the mothods were underesti-mated in varying degrees(1.4%-10.3%).2, The volumes calculated by Simpson's rule correlaled more closelywith the real volume than those calculated by area-length formula and the difference is signifieant (P<0.05).3.Areas obtaincd from the above four ways were significantly different (P<0.05).Conclusion : Left ventricular model volume can be assessed by ABD technique and real volume will be under-estimated. The method of tracink the ROI has an influence on the results.
