摘 要:犬在失血休克5h内,PaO_2波动上升,PaCO_2进行性降低。用PvO_2和PvCO_2反映细胞用氧状态发现,在休克1st至3rdh在心房及诸器官有进行性PvO_2降低和PvCO_2升高,在3td至5thh二者相对恒定。此时从血氧解离曲线看,静脉氧饱和度和张力仍超过代偿极限1~2倍,代偿储备远未耗竭,氧的供求平衡是一种假象,实为细胞有用氧障碍所致。将诸器官作双因素方差分析发现肝PvO_2最低PvCO_2最高,而肾则相反,变化反映了器官的解剖生理特点,肾PvCO_2从3rdh方急剧上升,提示开始有肾排酸保硷功能受损。在5h实验全过程,动脉血pH略高于静脉血pH并平行地进行性下降,未随血气稳定而不变,说明它不是缺氧引起。In the course of 5h after hemorrhagic shock in dog, PaO_2 rose wavilywhile PaCO_2 fell progressively. Using PvO_2 and PvCO_2 as an index to reflect the O_2 utili-zation status of cell, the blood of central vein and various organic vein were tested. Theresults were as follows: PvO_2 fell and PvCO_2 rose progressively in the 1st to the 3rd hafter shock, then, within 3rd to 5th h each of them came into relatively stabilized. In themean time, the oxygen saturation and O_2 tension of venous blood Were 1 to 3 tims abovethe compensatory limit according to oxygen dissociation curve. Compensatory storage hadnot be exhausted. The equilibrium between supply and demand was false appearance whichmeans that the cell was not able to utilize oxygen. Using two factor variance analysistreating the mean value of PvO_2 and PvCO_2 of organs results were as follows: the PvO_2in liver was lowest and the PvCO_2 was highest, while in kidney it was on the contrary,which appeared to reflect its difference in anatomico-physiological characters. The renalPvCO_2 rose rapidly from the 3rd h which showed renal function insufficiency. The pH ofarterial blood was higher than that of venous, they decreased continuously alI throughthe course of shock.The blood pH parting with blood gases stabilizing showed that acid-osis was not induced by hypoxia.
