摘 要:目的比较常规剂量普通肝素(UFH)和低分子肝素(LMWH)在先天性心脏病心导管术中应用的安全性及有效性。方法选择1999年8月至2004年5月共68例行心导管造影或心导管术患儿,UFH组穿刺成功后给予100U/kgUFH行全身肝素化。LMWH组于术前3小时给予1mg/kgLMWH皮下注射。于术前、术后5、15、30分钟、手术完毕分别取血测激活凝血时间(ACT)、部分激活凝血活酶时间(APTT)和抗a生物学活性。结果68例患儿均未出现出凝血事件,两组比较LMWH组比UFH组平均抗a活性稳定。结论先心病心导管术中应用LMWH安全而且有效的。Objective To contrast the safety and efficacy of the usual dosage unfractionated heparin(UFH) and low molecular weight heparin(LMWH), which were used in pediatric cardiac catheterization of congenital heart deisase.Methods 68 patients with pediatric cardiac catheterization were chosed and they were divided into two groups.The UFH group was treated with UFH of 100U/kg after successfully punctured,and the LMWH group was subcutaneously administrated 3 hours before catheterization at a dosage of 1mg/kg.Blood samples for coagulation studies by measuring ACT,APTT and anti-Ⅹa activity were obtained before operation,5,15 and 30 min after puncture,and at the end of heart catheterization.Results All of 68 children that had no bleeding or thrombosis.The mean anti-Ⅹa levels of the UFH group had a greater varition than the LMWH group.Conclusion It is safety and efficacy that subcutaneously administrated LMWH in the pediatric heart catheterization of congenital heart disease.
