尿NMP_(22)检测诊断膀胱癌的价值

摘　　要：目的　探讨尿核基质蛋白(NMP2 2 )检测对膀胱癌的诊断价值。方法:采用酶联免疫法测定38例初发性膀胱癌(观察1组) ,2 0例复发性膀胱癌(观察2组)及2 0例健康人(对照组)尿NMP2 2 水平。初发性膀胱癌组中30例在术后7天再复查1次。结果　观察1组尿NMP2 2 的中位数为2 8.3×10 3U/ L,观察2组为14 .5×10 3U/ L,对照组为2 .6×10 3U/ L;两组膀胱癌患者尿NMP2 2 含量明显高于对照组(P <0 .0 1) ,且随肿瘤病理分级的增加而升高(P <0 .0 1) ;观察1组30例术后复查尿NMP2 2 ,结果较术前明显降低。膀胱癌患者尿NMP2 2 检测阳性率为70 .6 9% ,尿脱落细胞检测阳性率为17.74 % ,两者差异有显著性(P <0 .0 1)。结论　尿NMP2 2 对膀胱癌的诊断有较高的敏感性,可在一定程度上反映肿瘤的恶性程度和预后。Objective To assess the diagnosis value of urinary level of nuc lear matrix protein 22(NMP 22)in bladder cancer.Methods Urinary NMP 22 were determined in 38 patients with original blander cancer (study group 1),20 recurrent blander cancer (study group 2) and 20 healthy volunteers (normal grou p)by enzyme-linked immunosorbent assay.30 patients in study group 1 were deter m ined for urinary NMP 22 at 7 days postoperatively again.Results The media n NMP 22 in study group 1 was 28.3×103U/L and group 2 was 14.5×103U /L,both significantly higher than healthy volunteers 2.6×103U/L(P<0. 01). The NMP 22 of patiens in group 1 and group 2 elevated with tumors pathologi c grades increasing.30 patients in grpup 1 determined again postoperatively,an d the median urinary NMP 22 value decreased remarkably in comparison with p reoperation.The positive rate of NMP 22 in urine of study group 1 and 2 (7 0.69%) was significantly higher than that of cytological examination(17.74%)(P<0.01).Conclusion Urinary NMP 22 is a high sensitivity tumor marker for the diagnosis of blander cancer and can indicate tumor malignancy degree and pr ognosis.
