冠状动脉介入术后应用Angioseal血管闭合器发生出血的因素分析

摘　　要：[目的]分析经皮冠状动脉介入诊疗术后使用Angioseal血管闭合器发生出血的影响因素,探讨应用该闭合器后应引起注意的问题。[方法]选取经皮冠状动脉造影或冠状动脉介入治疗后通过股动脉造影应用Angioseal血管闭合器病人70例,对发生出血因素进行Logistic回归分析。[结果]应用Angioseal血管闭合器后发生出血16例,出血与患高血压、大量吸烟以及围手术期应用肝素及低分子肝素有关。[结论]Angioseal血管闭合器止血效果较好,并发症少,应用于高血压者要特别注意观察,术后应用肝素及低分子肝素发生出血的几率增加,可延长卧床时间。Objective: to analyze the influencing factors of postoperative bleeding after using Angioseal blood vessel closer for patients underwent percutaneous coronary artery intervention, to probe into problems ones should pay attention to after applying of Angioseal blood vessel closer. Method: a total of 70 patients underwent percutaneous coronary arteriongraphy or percutaneous coronary intervention therapy applied Angioseal blood vessel closer were selected and the bleeding factors of them were analyzed with Logistic regression analysis. Result: totally 16 cases had developed postoperative bleeding after using Angioseal blood vessel closer, which associated with hypertension, hard smoking and using of heparin or low molecular heparin in peri-operative period.Conclusion: it showed that Angioseal blood vessel closer has a good effect on preventing bleeding and reducing complications. However, one should pay more attention to those hypertension cases. They would be observed closely after surgery. The bed-rest time of cases using heparin or low molecular heparin after operation would be prolonged.
