摘  要:目的探讨自知力教育对预防首发精神分裂症患者复发的影响。方法将110 例首发精神分裂症患者随机分为自知力教育联合利培酮治疗组(观察组,56例)及单用利培酮治疗组(对照组,54 例)。分别于入院和出院时进行阳性与阴性症状量表(PANSS)、自知力与治疗态度问卷(ITAQ)及服药依从性评定;对两组临床疗效达显效以上出院的患者进行为期2年的随访,以了解其复发情况。结果2 次PANSS评分两组间比较,差异均无显著性意义(均P>0.05);ITAQ评分入院时两组间差异无显著性意义(P>0.05),但出院时观察组ITAQ评分明显高于对照组(P<0.01);服药依从性入院时两组间差异无显著性意义(P>0.05),但出院时观察组服药依从性明显优于对照组(P<0.05)。2年内,观察组复发率为26.3%,明显低于对照组的54.3%(P<0.05)。结论对首发精神分裂症患者实施自知力教育,可提高其自知力和服药依从性,并降低其复发率。Objective To explore the effect of insight education in preventing relapse in first episode schizophrenic patients. Methods One hundred and ten inpatients with first episode schizophrenia were randomly assigned into two groups: insight education and risperidone therapy group (control group, n=56). risperidone therapy group (observation group, n=54), The Positive and Negative Symptom Scale (PANSS), the Insight Treatment Attitude Questionnaire (ITAQ) and drug therapy compliance were used to assess the patients in above two groups just when they were admitted in and discharged from hospital. These patients who had been discharged from hospital after recovery or marked progress were followed up for two year. Results The scores of PANSS in two groups didn’t show significant difference (P>0.05). The scores of ITAQ had no significant difference at the beginning of hospitalization between two groups, but the score in observation group was significantly higher than that in control group when they were discharged (P<0.01). The compliance of drug therapy in observation group was significantly better than that in control group (P<0.05). The rate of recurrence in observation group was significantly lower than that in control group within two year (P<0.05). Conclusion Insight education for first episode schizophrenia might be effective in relieving symptoms and reducing relapse rate.
