纳洛酮防治45例新生儿缺氧缺血性脑病临床疗效观察

摘　　要：目的探讨早期应用纳洛酮防治新生儿缺氧缺血性脑病的疗效。方法将2002年5月～2004年5月收治的窒息复苏后新生儿及窒息复苏后新生儿缺氧缺血性脑病患儿45例为治疗组,将2000年4月～2002年4月收治的同种患儿56例作为对照组,两组患儿均采用吸氧、纠正酸中毒、止惊、抗感染、降颅压、抗氧化剂、脑细胞营养和保护、能量合剂并辅以高压氧治疗,治疗组在此基础上加用纳洛酮(剂量0.2mg加入10%葡萄糖30ml,以10ml/h的滴速静滴,每日1次,连用5日)治疗。结果治疗组显效21例,有效17例,无效7例。对照组显效18例,有效20例,无效18例,两组比较差异有显著性。结论纳洛酮对于新生儿缺氧缺血性脑病有显著疗效,无明显不良作用,可常规用于防治新生儿缺氧缺血性脑病。Objective:To study the effect of naloxone used early in the pr ev ention and treatment of hypoxic ischemic encephalopathy in neonates.Methods:45ne onates with hypoxic ischemic encephalopathy after asphyxia resuscitation fromMay 2002to May2004treated in Longgang central hospital of Shenzhen were served as tr eatment group,while56neonates with the same disease from2000to2002as control gr oup.The neonates of hypoxic ischemic encephalopathy in both groups were given wi th correcting acidosis,anti-convulsion,anti-infection,dehydration and decreas ing intracranial pressure,antioxidant,cerebrolysin,citicoline,energy mixture assisted with hyperbaric oxygen therapy.0n this basis,the treatment group also received naloxone0.2mg in10%glucose30ml iv drip10ml/h once a day for5days.Resul ts:The significant effect,effecˉtiveness and inefficacy in the treatment and c ontrol group were21,17,7cases and18,20,18cases,respectively,with a signifi cantly difference between the two groups.Conclusion:Naloxone has significant eff ect on neonates with hypoxic ischemic encephalopathy without obvious side efˉfe cts.It may be used as a routine drug for treating hypoxic ischemic encephalopath y in neonates.
