摘　　要：目的探讨开放性肾手术结合气压弹道碎石术治疗肾复杂铸型结石的手术疗效。方法总结23例采用肾窦内肾盂加肾后下部切开结合气压弹道碎石术治疗肾复杂铸型结石患者的临床资料及经验并进行分析。结果23例患者均取石成功。术中不阻断肾蒂血流,手术平均时间138min,术中平均出血130ml,术中需输血者7例,平均输血量300ml,无术后继发出血等并发症。18例获随访,术后3个月B超检查见肾内残留小结石2例,直径<0.8cm。结论该术式操作简便,出血少,对肾功能损害小,疗效满意。Objective To clarify the effectiveness of intrarenal surgery supplemented with pneumatic lithotripsy for treatment branched calculi. Methods 23 cases clinical data and experience who had been treated with an incision of the intrarenal sinus and a postrenal low poie segmental incision supplemented with preumatic lithotripsy were analyzed retrospectively. Results Lithotomy was successful in all cases with no interncption of renal pedical and the average operation time 138 min. Blood transfusion during the operation was necessary in 7 cases with an average of 300 ml. Postoperative examiration of 18 cases with B ultrasound revealed residual calculi in 2, with diameters being less than 0.8cm. Conclusions The operation has the advantages of simple, less bleeding, preservation of renal function and a good efficacy.
