摘  要:目的 探讨用超声微泡造影剂促进VEGF基因治疗兔下肢缺血的可行性。方法 将30 只大白兔左股动脉结扎后随机分为3组: A组采用局部注射VEGF质粒DNA; B组采用局部注射VEGF质粒与超声微泡造影剂的混合物;C组作为对照。于治疗后4周进行数字减影血管造影(DSA)和免疫组织化学方法检测侧支循环建立和血管新生。结果 DSA可见用超声破坏微泡后促进侧支循环建立较好,新生血管较多;单纯局部注射质粒可促进部分血管新生,而对照组的侧支循环和新生血管较少。各组免疫组化所测血管计数均有显著性差异。结论 用超声微泡介导的VEGF基因转染,可促进缺血骨骼肌的血管新生和侧支循环建立,为下肢缺血性疾病的基因治疗提供了一种新途径。Objective To investigate the feasibility of microbubble-mediated VEGF gene delivery to treat rabbit's hind limb ischemia resulted from acute artery occlusion. Methods Thirty rabbits with the left femoral artery ligation were randomly and averagely devided into 3 groups: the plasmid pcDNA3.1/VEGF165 local intramuscular injection group (group A), the plasmid and microbubble mixture local intramuscular injection plus 1 MHz, 2.0 W/cm 2 ultrasound local radiation group (group B), and the control group (group C). Four weeks post-treatment, digital subtraction angiography (DSA) and immunochemical staining were applied to evaluate the collateral circulation of the treatment region. Results In the collateral circulation of the treatment region, both the angiographic score and the numbers of capillaries per high power field showed that the group B was superior to the group A, the group A superior to the group C, and there were significant differences between groups. Conclusion Microbubble-mediated VEGF165 plasmid therapy could promote collateral development and new vessels in the ischemic skeletal muscles, this provides a new method for the treatment of limb ischemia diseases.
