摘 要:目的 探讨系统性红斑狼疮中枢神经系统病变的MRI表现。方法 60 例原发性系统性红斑狼疮中枢神经系统病变的患者进行MRI检查。结果 原发性系统性红斑狼疮中枢神经系统病变主要表现为脑部皮层下斑点状和片状病变、脑梗死、出血、基底节区对称片状病变、桥脑中央髓鞘溶解症和脑萎缩。脊髓受累表现为横断性脊髓炎、纵行性脊髓炎和MS样改变。片状病灶的患者临床更易发现定位体征(χ2=15.74, P=0.001)。结论 MRI检查有助于明确病变性质,更准确地评估病情和预后,从而指导治疗和评价疗效。Objective To analyze the MRI manifestations of central nervous system involvement in systemic lupus erythematosus. Methods Sixty patients who fulfilled the diagnostic criteria of primary central nervous system involvement of systemic lupus erythematosus were examined with MR imaging. Results The MRI features included subcortical focal and diffuse lesions, infarcts, hemorrhage, symmetrical diffuse lesions in bilateral ganglions, central pontine myelinolysis and cerebral atrophy. Transverse myelitis, longitudinal myelitis and 'MS-like' lesions were found in the spine cord involved. The local physical symptoms was ascribed to the increase of patches lesion ( χ 2 =15.74, P =0.001). Conclusion MR imaging in central nervous system involvement in systemic lupus erythematosus can reveal the lesion's characteristics and evaluate the state and prognosis of the illness, and it is important in guiding the therapy and evaluating the response to treatment.
