摘 要:目的:用显微外科技术,模拟小鼠原位肝移植中移植肝的动脉血供,观察肝动脉的重要性。方法:40只BALB/C小鼠随机分成A,B两组,分别模拟无肝动脉重建和有肝动脉重建的原位肝移植手术,观察两种小鼠模型中移植肝的血供。A组分离结扎肝固有动脉,除肝十二指肠韧带外,其余的肝周韧带给予缝扎,但不予切断,以阻断除来自肝周的血供。B组除不结扎肝固有动脉外,其余手术步骤与A组相同。术后观察对比小鼠存活率、肝功能和肝组织学改变。结果:40只小鼠,手术成功率为92.5%(37/40),有3只因麻醉及术中误伤大出血而死亡(A组2只,B组1只),其余37只均手术成功。术后两周内生存率A组为55.5%(10/18),B组为100%(19/19)(P<0.05)。A组肝功能检查明显异常,谷草转氨酶(AST)和碱性磷酸酶(AKP)与B组比较升高明显(P<0.05)。A组肝组织学有明显的缺血表现,而B组肝功能和组织学检查均正常。结论:肝动脉吻合在小鼠原位肝移植模型的制作中具有重要意义,可明显提高该模型的稳定性。Objective:To study the importance of the anastomosis with microsurgery techniques in orthotopic liver transplantation in mice. Methods: 40 BACB/C mice were divided randomly into A、B groups. They were operated to mimic the liver blood supply in the two models of nonarterialized and arterialized orthotopic liver transplantation. In group A: the proper hepatic artery and the peripheral ligaments of the liver(except the hepatic duodenal ligament)were dissected, sutured and ligated. But they were not cut off. In group B, the surgical procedure was the same with the group A, except the ligation of the proper hepatic artery. The function and the histological changes of the liver were examined and compared after the operation. The survival rate was also compared. Results: 3 cases died of hemorrhage during the operation and anesthesia; The operation successful rate was 92.5%(37/40). The serum AST levels, especially the serum alkaline phosphatase levels of group A were increased significantly. In contrast, they remained normal in group B(P<0.05). The liver ishemic changes were found in group A, but not in group B. The survival rate in two weeks after the operation group A, was 55.5% (8/18), group B, was 100% (19/19)(P<0.05). Conclusion: It is necessary and feasible to anastomose the hepatic artery in establishing model of orthotopic liver transplantation in mice.
