摘  要:目的:骨的形成、骨折的愈合和骨的改建过程中均有生长因子参与,本文旨在探讨血管内皮生长因子在骨形成骨愈合过程中的作用。方法:实验于2004-06/09在吉林大学第一医院骨科研究所完成。选用SD大鼠40只制作骨缺损模型,随机分为血管内皮生长因子组、骨形态发生蛋白4组、血管内皮生长因子+骨形态发生蛋白4组,血管内皮生长因子多克隆抗体+骨形态发生蛋白4组,每组10只。将充满着血管内皮生长因子、骨形态发生蛋白4、血管内皮生长因子+骨形态发生蛋白4的7mm明胶海绵条分别置入该组骨弧形缺损处,血管内皮生长因子多克隆抗体+骨形态发生蛋白4组在骨缺损模型建立后,腹腔注射血管内皮生长因子多克隆抗体400ng,每3天1次,共3周。术后第3,6周分别选取5只拍摄X射线片观察骨形成情况,同时取骨缺损处标本光镜检查其病理改变。结果:实验过程中40只大鼠无死亡,均进入结果分析。影像学显示在第3周血管内皮生长因子组骨缺损处没有骨形成;骨形态发生蛋白4组和血管内皮生长因子+骨形态发生蛋白4组的5只大鼠骨缺损处均有骨形成,血管内皮生长因子+骨形态发生蛋白4组的骨形成比骨形态发生蛋白4组范围大、密度高;血管内皮生长因子多克隆抗体+骨形态发生蛋白4组骨缺损处的骨形成范围较骨形态发生蛋白4组少。AIM:Growth factors are involved in bone formation,fracture healing or bone reb uilding.This study aimed to investigate the effect of vascular endothelial growt h factor(VEGF) in bone formation and healing. METHODS:This experiment was done in the Department of Orthopaedics,First Affil iated Hospital of Jilin University from June to September 2004.Models of bone de fect were established in 40 SD rats,and then they were randomly divided into fou r groups:VEGF treatment group,bone morphogenic protein 4(BMP4) treatment group,V EGF+BMP4 treatment group and anti VEGF+BMP4 treatment group,with 10 rats each .Gelfoam disk 7 mm filled with VEGF,BMP4 and VEGF+BMP4 was transplanted into th e arc defects of corresponding groups.VEGF polyclonal antibody 400 ng was inject ed into abdominal cavity of rats in the anti VEGF+BMP4 treatment group after t he models of bone defect were established.All the intervention was performed eve ry 3 days for 3 weeks totally.Five rats were selected at 3 and 6 weeks after ope ration respectively,followed by radiography to observe bone formation;Meanwhile, the samples of bone defect were taken to observe pathological changes under ligh t microscope. RESULTS:No death was found in the 40 rats during the experiment.Radiography sh owed that VEGF alone could not improve bone healing,but either BMP4 or BMP4+VEG F induced bone formation in bone defects at 3 weeks.The bone formed in the BMP4 +VEGF group was significantly larger and denser than that in the BMP4 group at 3 weeks.The area of bone formation was smaller in the anti VEGF+BMP4 group tha n in the BMP4 group,and much smaller than in the VEGF+BMP4 group.A similar tren d was observed at 6 weeks among the 3 groups.It was found that under light micro scope at 3 weeks that there was no calcium deposition in VEGF group,but much cal cium deposition in BMP4 group;as compared with the BMP4 group,calcium deposition was increased in the BMP4+VEGF group,but decreased in anti VEGF+BMP4 group.S imilar results were found under microscope at 6 weeks.
