肝细胞肝癌对机体细胞免疫的影响

摘　　要：目的　探讨原发性肝细胞肝癌(HCC )对机体细胞免疫机能的影响。方法　采用血常规检测,应用荧光标记抗体及三色激光流式细胞仪对5 1例肝细胞肝癌患者和17例对照样本术前、术后外周血的细胞免疫指标进行检测。检测项目包括:T辅助淋巴细胞(THL) ,细胞毒T淋巴细胞(CTL) ,NK细胞,单核细胞,并进行手术前后的对比分析。结果　原发性肝细胞肝癌(HCC)术后THL、CTL、NK和单核细胞百分比、单核细胞绝对值的平均值较术前分别增高0 .91% ,8.5 8% ,8.85 % ,3 1.45 % ,66.5 5 % ,手术前后有显著性差异(P <0 .0 5或P <0 .0 1)。良性对照组的术前与术后上述各项指标比较,均无显著性差异(P >0 .0 5 )。结论　肝细胞肝癌(HCC)对机体的细胞免疫产生明显抑制作用,在肿瘤切除之后,这种抑制能力随之减弱,细胞免疫得到恢复。而良性对照组对机体细胞免疫的抑制不显著。综合分析细胞免疫的变化情况,对判断HCC的预后及肿瘤复发有重要的意义。Objective To investigated the responses of cellular immune system in hepatocellular carcinoma(HCC).Methods The peripheral blood were examined for cellular immune marker by the technique of fluorescence-marker antibody and three-color flow cytometry in 51 cases of HCC,17 cases of control before and after operation.The T helper lymphocytes(THL),Cytotoxic T lymphocytes(CTL),Natural killer(NK) cells and Mononuclear cells of peripheral blood were counted and analyzed for comparison study of the immune before and after operation.Results In HCC,the postoperative T helper lymphocytes,Cytotoxic T lymphocytes,NK cells and Mononuclear cells were increased by 0.91% ,8.58%,8.85%,31.45%,and 66.55%,respectively(P<0.05,P<0.01),showing a significant difference.No significant difference was found in cellar immune markers between the preoperative and postoperative patients with benign control.Conclusion The cellular immune system response was depressed in patients with HCC.It will be recover after operation.However,There is no marker inhibitory effect on the cellular immune in patients with benign control.Comprehensive analysis of those cellular immune markers has great significance for the prognosis and determination of the recurrence in HCC.
