摘  要:目的:探索玫瑰糠疹的发病机制。方法:采用免疫组化法检测玫瑰糠疹皮损血管内皮细胞的细胞间黏附分子(ICAM)-1的表达,通过透射电镜观察玫瑰糠疹皮损血管内皮细胞超微结构的改变。结果:血管内皮细胞ICAM-1的表达皮损组均高于正常皮肤组及玫瑰糠疹皮损周围皮肤组,而且玫瑰糠疹皮损周围皮肤组高于正常皮肤组,在统计学上差异均有显著性(P<0.05)。5/8标本中可见真皮血管内皮细胞连接处间隙扩大,紧密连接消失或破坏。结论:ICAM-1在玫瑰糠疹皮损血管内皮细胞的高表达及玫瑰糠疹真皮血管内皮细胞超微结构的改变,可能与玫瑰糠疹部分皮损内可见数量不等的血管外红细胞有关。Objective: To study the pathogensis of pityriasis rosea. Methods: The expression of ICAM-1 on the endothelial cells of blood vessels of the skin lesions was detected by immunohistochemistry, and the ultrastructures were observed by transmission electron microscopy. Results: The results showed that the expression of ICAM-1 on the endothelial cells of blood vessel in the lesions was higher than that around the lesions as well as in the normal skin. Besides, the expression of ICAM-1 on the endothelial cells around the lesion was also higher than that in the normal skin(P < 0.05). There were enlarged gaps between the conjunctions of epithelial cells, and tight conjunctions disappeared or damaged in 5 out of 8 cases. Conclusion: The higher expression of ICAM-1 in the lesions and the ultrastructural changes in the conjunctions of the endothelial cells of blood vessel is probably related to the exocytosis of erythrocytes.
