摘  要:目的:探讨99mTc标记葡萄糖(99mTc-EC-DG)在肿瘤显像中的可行性。方法:EC-DG标记时加入氯化亚锡及新鲜淋洗的99mTcO4-,振摇,静置后即完成标记。薄层色谱仪(TLC)测定标记率。2例肺癌、1例淋巴瘤、1例胃癌术后复发、1例结肠癌术后复发、1例甲状腺癌肺转移、1例肺炎和1例肺结核患者于空腹状态下静脉注射99mTc-EC-DG25m Ci后2、4h显像,并计算T/N比值,6例恶性病变患者中4例做了18F-氟脱氧葡萄糖(18F-FDG)对比显像,同样计算T/N值。结果:99mTc-EC-DG显像时脑组织未见显影,两肾放射性较高;与18F-FDG相比,99mTc-EC-DG的血液清除较慢,2h时心脏、大血管影像仍可见,4h才基本消退;肌肉组织摄取放射性少。6例恶性病变的显像结果为阳性,2h T/N比值1.95~5.64,4h T/N比值2.05~6.88,2例良性病变的显像结果为阴性;18F-FDG显像时4例恶性病变患者结果均为阳性,T/N比值5.80~12.35。结论:99mTc-EC-DG用于恶性肿瘤的葡萄糖代谢显像是可行的,但目前尚不能完全替代18F-FDG显像。Objective To evaluate the feasibility of 99mTc labeled glucose 99mTc-EC-DG in tumor imaging. Methods 99mTc-EC-DG was achieved by means of adding the required amount of EC-DG and tin (Ⅱ) chloride to the pertechnetate. Radiochemical purity was assessed at radio-thin-layer chromatography. After 6 h fasting, eight patients induding 2 with lung cancer,1 with lymphoma,1 with recurrent gastric cancer,1 with recurrent of colon cancer,1 with lung metastasis of thyroid cancer, 1 with pneumonia and 1 with tuberculosis,underwent the 99mTc-EC-DG imaging after injection of 25 mCi 99mTc-EC-DG intravenously. The planar and tomographic imaging was acquired at hours 2 and 4 after injection, and the ratio of tumor to normal tissue was calculated. Four of the 6 cases of malignant tumor were performed 18F-FDG contrast imaging, and the T/N ratio was also calculated. Results The brain was not imaged while the kidneys were clearly imaged in 99mTc-EC-DG imaging. The blood clearance of 99mTc-EC-DG was slower than that of 18F-FDG. The blood pool of heart and big blood vessel was seen at hour 2 after injection of 99mTc-EC-DG, and was still visible at hour 4. The uptake of muscle was low. Six cases of malignant tumor had positive imaging results, and the T/N ratio at hour 2 was 1.95-5.64 and that at hour 4 was 2.05-6.88. Two cases with benign lesion had negative results. Four cases of malignant tumor had positive results of 18F-FDG imaging with the T/N ratio of 5.80-12.35. Conclusions 99mTc-EC-DG tumor imaging for glucose metabolism is feasible, just like 18F-FDG imaging, which can diagnose the tumor and monitor the therapeutic response, but it is necessary to study more cases of all kinds of cancers to determine its value in clinical application.
