血小板参数变化与早产儿颅内出血的相关性

摘　　要：目的　研究血小板参数变化对早产儿脑室周围 脑室内出血的影响 ,探讨其临床意义。方法　检测 80例脑室周围 脑室内出血早产儿血小板参数变化 ,并与 5 5例无脑室周围 脑室内出血的早产儿和 4 0例正常足月新生儿各项指标相比较。结果　血小板计数分别为 (2 99.6± 80 .5 )× 10 9 L、(2 39.5± 86 .5 )× 10 9 L和 (175 .7± 78.8)× 10 9 L ,血小板比积分别为 (0 .2 5 4± 0 .0 9) %、(0 .189± 0 .0 9) %和 (0 .139± 0 .0 6 ) % ,提示脑室周围 脑室内出血患儿血小板计数和血小板比积显著降低 (F =31.84 ,P <0 .0 1)。而血小板平均容积与血小板分布宽度差异不显著 (P >0 .0 5 )。结论　血小板参数的异常变化可能参与了早产儿脑室周围 脑室内出血的发病机制 ,要重视对早产儿凝血状态的监测 ,以减少其发生。Objective To study correlation of changes in parameters of plat elet with periventricular-intraventricular hemorrhages(PIVH) in premature infan ts and its clinical significance. Methods The parameters of plat elet including platelet count(PLT,×10 9/L), thrombocytocrit(PCT,%),mean pl atelet volume(MPV,f l) and platelet distribution width(PDW,%) were examined respectively i n 80 premature infants with PIVH and 55 premature infants without PIVH and 40 he althy term infants. Results As compared with term infant s and premature infants witho ut PIVH, PLT and PCT of infants with PIVH were lower significantly(PLT: 299.6±8 0.5×10 9/L, 239.5±86.5×10 9/L and 175.7±78.8×10 9/L,respectively; PCT: 0.254 ±0.09%, 0.189±0.09% and 0.139±0.06%. F=31.84,P<0.01), while MPV and PDW were similar in all groups(P>0.05)。Conclusion The chang es in parameters of platelet may involve the pathogenesis of PIVH in premature i nfants, and more attention should be paid in monitoring of coagulation status of premature infants so as to reduce the incidence of PIVH.
