55例女性逼尿肌无力症的诊治

摘　　要：目的探讨女性逼尿肌无力症的临床诊断及治疗方法。方法针对主诉排尿困难,尿动力学检查结果主要表现为逼尿肌乏力的女性患者,进一步从病史、临床检查等多方面尽可能地寻找逼尿肌乏力的临床病因,排除膀胱出口梗阻等器质性病变。根据病因做相应治疗并观察疗效。结果55例逼尿肌无力症中有明确病因者20例,其中糖尿病8例,解痉剂应用7例,膀胱过度扩张5例。另外,伴有明显心理情绪障碍者28例。结果34例完全治愈,18例部分治愈,3例无效。结论女性逼尿肌无力症的诊断主要依靠尿流动力学检查,部分患者有明确的病因,心理情绪障碍也可能是病因或疾病表现之一。针对病因做综合治疗有较好效果。Objective To investigate clinical cause of disease and therapeutic means of female detrusor underactivity.Methods For the female patients who chief complain was dysuria and results of urodynamic examination manifested primarily detrusor underactivity, to find as far as possible clinical cause of disease of detrusor underactivity from medical history,clinical examination and other aspects ,and exclude organic pathologic change,for example,bladder outlet obstruction and so on .To carry out appropriate treatment and observe curative effect according to cause of disease.Results The patients had definite cause of disease in 20 of 55 patients with detrusor underactivity.The 20 cases with definite cause of disease included 8 cases of diabetes,7 cases of antispasmodic used and 5 cases of bladder with over dilatation. Results after treatment suggested 34 cases cured completely , 18 cases of improvement of symptom , and 3 failed to gain effect.Conclusion Diagnosis of female detrusor underactivity depended on mainly examination of urodynamics.Partial patients had definite cause of disease . But the patients with detrusor underactivity had much more patients of concomitant obvious psychological block .Performing comprehensive treatment on this cause of disease had fine effect.
