股四头肌滑动装置的形态及其临床意义

摘　　要：在２５具尸体标本上对股四头肌周围的结缔组织、股四头肌各肌间的肌间隙、骸上囊及其深面的脂肪垫进行了观测。提出股四头肌滑动装置的概念，此滑动装置包括股四头肌周围的结缔组织、股四头肌各肌间的肌间隙、髌上囊及其深面的脂肪垫。股骨干骨折切开复位内固定术切口的选择以后外侧切口为首选，外侧切口次之。前外侧切口多损伤此滑动装置，引起股四头肌粘连及膝关节功能障碍，应放弃。On 25 cadavers,the slipping device of quadriceps femoris was investigated.It is composed ofthe connective tissue surrounding the quadriceps femoris,the spaces within the muscle,the supra-patellar pouch and the fat cushion. Clinically,the posterolateral operative approach is preferable inthat it does not seriously affect the slipping device,The anterolateral approach should be avoidedas it is through the vastus intermedius muscle,which may lead to adhesion of the structures con-cerned and consequently loss of knee functions.
