高血压病患者颈动脉超声检测的相关研究

摘　　要：目的探讨各级高血压病患者的颈动脉粥样硬化程度,以及与高敏C反应蛋白(hs -CRP)的关系。方法对140例高血压病患者与24例正常对照者行颈动脉超声检查,测量颈动脉内膜中层厚度(CAIMT),颈动脉内径(CADIA),并行常规体检及血生化检查。结果随着高血压分级的逐渐增加,CAIMT逐渐増厚;CAIMA逐渐扩大;斑块的发生率也逐渐增加,各级高血压分组与正常对照组比较,差异有显著性;hs- CRP逐渐增高;血液中总胆固醇(TC)、三酰甘油(TG)、低密度脂蛋白胆固醇(LDL C)、高敏C反应蛋白(hs CRP)、空腹血糖(FBG)、餐后2h血糖(PBG)均有上升趋势;高密度脂蛋白胆固醇(HDL C)则呈下降趋势。hs- CRP与斑块的个数呈显著正相关。结论血糖、血脂、hs- CRP水平可加重高血压患者颈动脉粥样硬化程度,hs- CRP测定结合颈动脉超声检测可以及早发现和诊断颈动脉斑块,评估动脉硬化的程度,有助于预测和预防急性心脑血管事件发生。Objective To study the ultrasonic characteristics of the carotid artery atherosclerosis in patients with hypertension, and discuss the relationship between the degree of carotid artery atherosclerosis and the level of high sensitive-C-Reactive-Protein (hs-CRP). Methods Color doppler ultrasonography of carotid arteries were used to exam 140 cases of hypertension patients (the first grade group 44,the second grade group 84,the third grade group 12) and for 24 cases of healthy volunteers as the control group. Results Among the hypertension group, intima-media thickness of common carotid artery (CAIMT) is thicker and carotid diameter (CADIA) is getting widening while the hypertension is getting worse. Also, the hs-CRP, TC(total cholesterol), TG(triglyceridemia), BG(blood glucose) was higher in the hypertension than that in the control group. The rates of carotid atherosclerotic plaques of hypertension were higher than that in the control group. Hs-CRP was positive correlated with the number of the plaques. Conclusion Hypertension, hypercholesterolemia and high blood glucose combined with the hs-CRP had effects on the progress of atherosclerosis. The early examination and diagnostic of atherosclerosis are helpful to predict and prevent the complication.
