妊娠期高血压疾病并发HELLP综合征29例临床分析

摘　　要：目的分析妊娠期高血压疾病并发HELLP综合征的临床表现及对母儿的影响,探讨有效的诊断和治疗方法。方法1996年1月至2004年3月中山市人民医院采用回顾性分析方法,对29例Ⅰ型和Ⅱ型HELLP综合征的临床资料进行分析,对比两组患者的临床表现、母儿并发症和分娩结局。结果Ⅰ型HELLP组的重度子疒间前期和子疒间发生率为94.7%,明显高于Ⅱ型HELLP组(50.0%)(P<0.05);Ⅰ型HELLP组出现特殊表现者占68.4%,亦高于Ⅱ型HELLP组(30.0%)(P<0.05);Ⅰ型HELLP组围生儿死亡率31.6%,Ⅱ型HELLP组围生儿死亡率10.0%,两组比较差异有显著性意义(P<0.05);两组间产妇并发症、剖宫产率比较差异无显著性意义(P>0.05)。两组患者血小板值产后72h恢复正常。结论早期诊断、综合性治疗、适时终止妊娠,可改善HELLP综合征患者的预后。Objective To analyze the clinical presentation and the effects on both mother and fetus wi th HELLP syndrome induced hypertensive disorder complicating pregnancy and to investigate effective methods of diagnosis and treatment. Methods The data of 29 patients with HELLP syndrome type Ⅰ and type Ⅱ from People's H ospital of Zhongshan City from 1996 to 2004 were analyzed retrospectively.Th e clinical presentations,maternal-neonatal complications and pregnant outcomes were compared between the two types. Results The incidence of severe pre-eclampsia,eclampsia and special presentations were significantly higher in type Ⅰ group than those in type Ⅱ group (94.7% vs 50 .0%;68.4% vs 30.0%,P<0.05).There were significant differences in the pe rinatal mortality between the two groups(31.6% vs 10.0%, P<0.05).No significant difference was found between the two groups in the inc idence of maternal complication and cesarean section rate (P>0.05).Normaliz ation of platelet counts were in 72 hours of postpartum in the two groups. Conclusion Better prognosis may be achieved by early diagnosis,synthetic therapy and opti mal termination of pregnancy.
