摘  要:目的 观察白细胞介素- 6 (IL - 6 )、IL - 10、肿瘤坏死因子-α(TNF -α)和转化生长因子- β1 (TGF -β1 )在充血性心力衰竭(CHF)患者中的变化及其意义。方法 用放免法测定4 9例CHF患者及30例健康体查者血清IL - 6、IL - 10、TNF -α,用ELISA测定TGF - β1 ,并进行分析比较。结果 (1)CHF组TNF -α、IL - 6、IL - 10水平明显高于正常组(P <0 .0 1) ,TGF - β1 水平明显低于正常组(P <0 .0 1)。心衰程度越重,变化越显著。(2 )TNF -α、IL- 6与LVEF呈负相关(r=- 0 .82 3、P <0 .0 0 0 1,r=- 0 .85 1、P <0 .0 0 0 1) ,而TGF - β1 与LVEF呈正相关(r=0 .814、P <0 .0 0 0 1)。结论 CHF时TNF -α、IL - 10和IL - 6水平升高,TGF - β1 降低,细胞因子网络处于失衡状态。TNF-α、IL - 10和TGF - β1 水平与心衰严重程度相关,可作为判断心衰严重程度的辅助指标之一。Objective To observe the changes of the levels of TNF-α,IL-6,IL-10 and TGF-β_1 in serum of the patients with CHF and to find new proper parameters for the monitoring of cardiac function.Methods 49 patients with CHF were selected and the serum levels of IL-6,IL-10,TNF-α were measured by radio-immunity assay,and TGF-β_1 by ELISA.Control subjects were 30 cases whose sex-and age-matched healthy blood donors.Results (1)The serum levels of TNF-α,IL-6,IL-10 were significantly increased in patients with CHF than that in healthy controls(P<0.001) along with increasing NYHA functional class(P<0.05).The TGF-β_1 was significantly decreased in patients with CHF along with increasing NYHA functional class(P<0.05).(2)LVEF was negatively correlated with the serum levels of TNF-α and IL-6(r=-0.823,P<0.000,r=-0.851,P<0.000), and positively correlated with TGF-β_1(r=0.814,P<0.000). Conclusion In patients with CHF,there are siganificantly elevated levels of cardiodamaging cytokines (TNF-α,IL-6) and raised(IL-10) or markedly decreased levels of cardioprotective cytokine (TGF-β_1).So the cytokine network is in dysbalanced situation. The serum levels of TNF-α,IL-6 and TGF-β_1 are significantly correlated with the degree of heart failure.
