胰管栓塞术在胰十二指肠切除术中应用的近远期疗效观察

摘　　要：本文报导１９８３年３月至１９９２年４月期间，４０例胰十二指肠切除术时采用胰管栓塞术的经验。用ＴＨ胶栓塞省略了胰空肠吻合步骤，缩短了手术时间；无胰瘘发生。通过空腹血糖、血浆胰岛素、葡萄糖耐量试验及胰岛素释放试验，证明残胰长期保持了正常内分泌功能。残胰活检见胰岛细胞密集，结构正常。作者认为胰管栓塞值得推广。0 pancreaticoduodenectomies with occlusive procedure of pancreatic ductwere performed between April 1983 and April 1992 in the authorsi hospital. Theocclusion substance was N-Octyl-Cyanoacrylate made in China. This methodsimplified the operating process by omitting pancreaticojejunostomy. The time ofsurgery was shortend- No pancreatic fistula occurred in this group. The longterm endocrine function of the residuary pancreas maintained after duct-occlusionwhich was proved by the normal values of the following tests : the fast blood glu-cose, the plasma insulin, the C-peptide, the glucose tolerance test and the in-sulin release test. The pancreatic stump biopsy showed that the islet cells weredense and in normal structure. The authors recommend that occlusive procedureof pancreatic duct can bewidely used for pancreaticoduodenectomy.
