^(13)-C尿素呼气试验检测幽门螺杆菌感染的评价

摘　　要：目的　探讨幽门螺杆菌(Helicobacter pylori,HP)感染3 种检测方法的差别及临床价值。方法　62 例胃十二指肠疾病患者同时行胃黏膜病理切片Giemsa染色、13C 尿素呼气试验(13C Urea breath test,13C UBT)、快速尿素酶试验(rapid ureasetest,RUT)检测HP感染。13C UBT、RUT检出HP的阳性率与病理切片Giemsa染色的检出HP阳性率进行比较。结果　40例慢性胃炎患者HP检测阳性率分别是Giemsa染色67 5%、13C UBT 60%、RUT 60%。22例消化性溃疡患者HP检测阳性率分别是Giemsa染色81 .8%、13C UBT 72 7%和RUT 68.1%。各检测方法HP检出阳性率之间统计学上差异无显著性。13C UBT和RUT诊断现症感染准确性分析为90 .3%、90. 3%,敏感性分别是94. 7%、92 .3%,特异性分别是83 3%、87. 0%,阳性预测值分别是90%、92 3%,阴性预测值分别是90 .9%、87. 0%。13C-UBT和RUT的准确性、敏感性、特异性、阳性预测值、阴性预测值之间在统计学上差异无显著性。结论　13C-UBT是一种准确性、特异性、敏感性高、无创性、无放射性的HP感染的诊断方法。Objective To explore the difference and clinic value of the three different detective methods for Helicobacter pylori infection.Methods 62 gastric and duodenal disease patients were studied.HP infection was detected with gastric mucosa pathobiology slice Giemsa staining,^(13)C-urea breath test and repid urease test at the same time.^(13)C-UBT and RUT results were compared with Giemsa staining HP positive rate.Results The positive rate of 40 chronic gastritis patients by three different methods was Giemsa staining 67.5%、^(13)C-UBT 60% and RUT 60%;The positive rate of 22 peptic ulcer patients by different test was Giemsa staining 81.8%、^(13)C-UBT 72.7% and RUT 68.1%.There was no statistically significant difference between the three methods.The correct rates for diagnosing HP infection of ^(13)C-UBT and RUT were 90.3% and 90.3%,with sensitivities 94.7% and 92.3%,the specificities 83.3% and 87.0% respectively.the positive predictive values 90% and 92.3%,and the negative predictive values 90.9% and 87.0% respectively.There was no statisticall difference between the two tests in the correct rate,the sensitivity,the specificity,the positive predictive value and the negative predictive value.Conclusion ^(13)C-UBT appears to be a high accurate,high specific,high sensitive,non radioactive and noninvasive test for the diagnosis of HP infection.
