子宫切除术后阴道上皮及血管的阴道镜像变化规律探讨

摘　　要：本文对130例绝经前因子宫肌瘤行子宫全切(卵巢切除或保留)术后随访2年的患者,用阴道镜观察阴道上皮与血管的变化,同时进行性激素测定、阴道涂片检查及询问更年期症状等来检测卵巢功能。并将阴道上皮血管变化与卵巢功能间的关系进行分析。同时与21例育龄妇女作对照研究。结果:33例阴道镜检查显示,阴道上皮表面完整、光滑,阴道无皱襞或皱襞极少,色苍白者称阴道上皮+,血管多数呈网状(30/33),为卵巢功能衰退、雌激素水平低落的阴道镜表现。80例阴道上皮表面完整丰厚,皱襞多、色淡红者称阴道上皮++,血管多数不清(75/80),是卵巢功能正常的阴道镜像。本研究结果提示,通过阴道镜像阴道上皮及血管的变化,可了解体内雌激素水平高低,扩大了阴道镜的使用范围。The purpose of the paper is to study the relation- ship between patterns of vaginal epithelium and blood vessels and the ovarinan function.One hundred and thirty cases were observed with colposcope for pat- terns of epithelium and blood vessels within 2 years after total hysterectomy.Ovarian functions were eval- uated by serum sex hormones,vaginal smear,and the climacteric symdrome.A method of grading the ep- ithelium was adopted,by which a vaginal epithelium which was complete,smooth,pale in colour,without or with very few folds was graded VE+;An epitheli- um which was complete,thick,with abundant folds, and light red in colour was graded VE++.Results showed that 33 patients were VE+.Most(30/33)of their vaginal epithelial blood vessels resembled net- work,sod their ovarian functions were low;80 pa- tients were VE++.Seventy-five of their epithelial blood vessels were indistinct,and their ovarian func- tions were normal.The rest 13 cases were excluded for lesions in the vaginal epithelia.We concluded that colposcopic imaging of the vaginal epithelium was an- other effective way of assessing the function of o- varies.
