颅脑手术使用阿法多芬在凝血机能方面的观察

摘　　要：目的:观察颅脑外科患者使用阿法多芬对凝血机能的变化。方法:择期行颅脑外科手术患者48例,随机分为观察组(A组)和对照组(B组)。A组于麻醉前静注阿法多芬注射液0.4(以100g/L葡萄糖注射液20ml稀释),B组则给予相同剂量的生理盐水。分别于术前、术中180min、240min取静脉血查凝血酶原时间(PT)、活化的部分凝血酶时间(APTT)、凝血酶时间(TT)、纤维蛋白原含量(Fbg)等凝血指标及监测两组生命征的变化(血压、呼吸、心率)。结果:B组PT、APTT、TT值随手术时间的延长明显增大,与术前比较差异显著(P<0.05)而A组各时点值变化不明显,与术前比较差异不显著(P>0.05)。纤维蛋白原含量(Fbg)两组变化不明显(P>0.05)。生命征两组比较无明显差异(P>0.05)。结论:颅脑外科患者麻醉前静注阿法多芬注射液0.4,可促进凝血、利于止血。可安全地应用于颅脑手术患者。Objective: To observe function on cruor for Afaduofen during craniocerbal operations. Methods: Fourty-eight patients were randomly divided into experimentation group(group A, n=24 cases) and control group(group B, n=24 cases). Patients in group A were administred with intravenous Afaduofen 0.4(diluted by 100g/L glucose) , patients in group B substitute for same volume of normal soline. Peripheral venous blood samples were taken before operation, 180min. and 240min.after skin incision for the levels including PT、APTT、TT、Fbg, vital signs were observed during the whole operation. Results: Compared with preoperation,the levels of PT、APTT、TT in group B were significantly increased(P<0.05),but the level of group A had little changed(P>0.05). The level of Fbg had little changed in the two group(P>0.05).In the two groups vital signs had little changed too(P>0.05). Conclusion: Afaduofen can promote cruor and is propitious to hemostasia for patients during craniocerbal operations. Wherefore,it is safety for patients during craniocerbal operations.
