男性2型糖尿病患者勃起功能障碍与性激素水平的关系

摘　　要：目的　探讨男性2型糖尿病患者勃起功能障碍与性激素水平的关系。方法　以问卷调查的方法比较60名男性2型糖尿病患者(观察组) 及51名男性非糖尿病者(对照组) 国际勃起功能指数(IIEF) 评分的差异, 并以化学发光法测定两组人群的血清性激素水平。结果　男性2型糖尿病患者IIEF评分及总睾酮(TT) 水平显著低于对照组(P值均为0. 000), 黄体生成素(LH) 水平显著高于对照组(P=0. 000)。在男性2型糖尿病患者中存在着随年龄的增加游离睾酮(FT) 水平、IIEF评分降低, 而LH水平升高的情况, 此现象在男性非糖尿病者中也存在, 但程度较轻, 发生年龄更大。FT同糖化血红蛋白(HbA1c) 呈负相关。结论　男性2型糖尿病患者的性腺功能及勃起功能减退, 睾酮水平与血糖水平呈负相关, 提示对男性糖尿病合并ED的患者有必要检测性激素以指导治疗。Objective To study the relationship betwe en erectile dysfunction(ED) and the abnormalities of the sex hormone levels in t ype 2 diabetes mellitus(T2DM) in men.Methods Erectile function(EF) of 60 men with T2DM and a con trol group consisting of 51 non-diabetic men were assessed by the International Index of Erectile Function(IIEF).The venous blood was drawn for the determinatio n of total testosterone(TT),estradiol(E 2),follicle-stimulating hormone(FSH),lu teinizing hormone(LH) and sex hormone binding goblin(HbA 1c ).Resu lts The IIEF scores and values of TT of diabetic men were significant ly lower than those of the controls(P=0.000,0.000) and the values of LH w ere significantly higher than those of the controls(P=0.000).There were sig nificant negative correlations between free testosterone(FT)values and IIEF sc ores with age in men with T2DM.This phenomenon also existed in non-diabetic men, but the degree was mild.In diabetic men,the FT value also negatively correlated to HbA 1cbesides age.Conclusion There are relative hypogonadism and ED in men wi th T2DM,and their FT values are negatively correlated to the chronic hyperglycem ia.It is very important to examine the sex hormone in men with T2DM with ED.The cause-effect relationship between T2DM and hypogonadism needs further research.
