妊娠末期合并血栓性肠坏死六例临床分析

摘　　要：目的　探讨妊娠末期并发肠系膜血栓形成致肠坏死相关因素及早期诊断的意义。方法　对我院近20年收治的6例妊娠末期并发肠系膜血栓形成肠坏死病例的临床资料进行回顾性分析。结果　6例患者均行剖腹探查术,并行坏死肠段肠切除肠吻合术, 治愈1例, 死亡5例。结论　该病发病急, 临床症状与体征不相符, 诊断困难, 不及时诊治可危及母儿生命。对晚期妊娠者, 出现腹部不适伴呕吐咖啡样物或剧烈腹痛, 而腹膜刺激征阴性时, 应迅速排除产科疾病, 立即按本病剖腹探查, 以便早期诊断与治疗, 减少病死率。Objective To study the diagnosis and the treatment of the intestine mecrsis led by the pregnancy with the mesentery vena thrombosis.Methods The causes of 6 intestine necrosis led by the pregn ancy with the mesentery venae thrombosis have been analyzed in recent 20 years i n our hospital.Results The six cases were carried out the l aparo-exploration and were all treated by the intestinal anastomosis with reseat ing the mecrosis intestine.Four were die and one was cured.Conclusion The disease occurs fast,the clinical symptom doe sn′t agree with the sign that will be diagnosed erroneously.It will endanger th e mother′s body and the fetal life,if it couldn′t be treated in time.When the pregnancy at the later period feels unwell in abdomen with the blood water vomit us or severe abdominal pain,and there is a peritoneum stimulated as a negative r eaction,the obstetrical disease should be forgotten swiftly and the laparo-explo ration as the above mentioned should be made at once so that the early diagnoses and treatment could be made and the death rate could be reduced.
