摘 要:目的:探讨原发性高血压(EH)患者24h平均脉压(ABPP)与心脏损害的关系。方法:将486例EH患者行动态血压检测的结果按ABPP≥60mmHg和ABPP<60mmHg分为两组,将两组的多普勒加二维超声心动图检测中主动脉内径、室间隔厚度、左心室舒张末期内径、左心室后壁厚度、左室重量、左室重量指数、EF、E/A,以及两组的心电图资料进行对比分析。结果:随着脉压增大,出现心电图异常病例增多,心肌肥厚、心脏扩大、主动脉内径增大病例增多,心脏收缩和舒张功能减退,两组数据经统计学处理差异有明显的统计学意义。结论:脉压增大,增加了对靶器官的损害,尤其是对心脏损害,进而造成心血管疾病发病率增高,因此在EH治疗中积极控制脉压将能有效减轻心脏损害。Obiective:To investigate the relationship between average 24-hour ambulatory pulse pressure and cardiac damage in hypertension patients. Method:Four hundred eighty-six patients with hypertension were enrolled into the following two groups by the mean values of average 24-hour ambulatory pluse pressure:ABPP≥60 mmHg (n=204) and ABPP<60 mmHg (n=282). The data of AO、 LVEDd、 IVSD、 LVPWd、 LVM、LVMI、EF and E/A were compared, at the meantime, the data of electrocardiogram were compared. Result:In the group with ABPP≥60 mmHg, the EF and E/A were significantly decreased, AO、LVEDd 、IVSD 、LVPWd and LVMI were increased, while electrocardiogram abnomality was more fvequent compared with the group with ABPP<60 mmHg. Conclusion:The increase of average 24-hour ambulatory pulse pressure plays important role in the process of left ventricular hypertrophy, cardiac function damage and electrocardiogram abnomality. Attention must be payed to decrease the average 24-hour pulse pressure in hypertension treatment.
