摘  要:目的探讨机械通气在急性有机磷农药中毒呼吸衰竭治疗中的作用。方法对1997年7月~2004年2月87例急性有机磷农药中毒(AOPP)合并呼吸衰竭的患者机械通气治疗的临床资料进行回顾性分析。结果87例AOPP合并呼吸衰竭患者应用呼吸机机械通气等综合救治措施后,抢救成功74例,成功率85%;死亡9例,死亡率10.4%;放弃治疗4例,占4.6%。机械通气时间1~11d,平均(3.6±1.4)d。结论快速清除体内毒物、尽早阿托品化、早期足量应用胆碱酯酶复能剂、合理使用机械通气是治疗AOPP并发呼吸衰竭的重要环节。Objective To investigate the treatment efficiency of mechanic al ventilation for respiratory failure caused by acute organophosphorus pesticid e poisoning (AOPP). Method Clinical data of 87 patients in our hospital treated with mechanical ventilation for the respiratory failure caused by acute organop hosphorus pesticide poisoning from July 1997 to February 2004 was analyzed retro spectively. Results 57 patients with respiratory failure caused by acute organ ophosphorus pesticide poisoning were treated with mechanical ventilation and oth er combined treatments. 74 patients had survived after the withdrawal of respira tory machine with a success rate of 85%; 6 patients died with a mortality rate o f 10.4 %; and two patients gave up treatment with a rate of 4.6%. The duration o f mechanical ventilation was 1~11 days,average(3.6±1.4)days. Conclusion Remov ing the poison inside the body and reaching atropinization as early as possible, administering enough cholinesterase reactivator at early stage and using mechan ical ventilation early and rationally are the important measures to treat respir atory failure caused by AOPP.
