全膝关节置换术后经皮神经电刺激即刻镇痛的临床对照研究

摘　　要：目的:探讨经皮神经电刺激(TENS)对全膝关节置换术后疼痛的镇痛作用。方法:42例患者随机分入两组,即TENS组和安慰刺激组。所有患者膝关节的术后疼痛程度采用视觉类比方法(VAS)进行评测。多项逐步回归分析法10.0版本比较TENS组与安慰刺激组前、中、后之间VAS的变化。结果:TENS组与安慰刺激组两组比较术后第一天疼痛程度(VAS)差异有显著性意义,TENS组明显降低术后疼痛程度。TENS组VAS评分从刺激前的100%降低至60min刺激中的83.3%±14.9%,及20min刺激后的85.7%±16.1%,并在TENS刺激后的60min回升到94.3%±7.8%。结论:全膝关节置换术后TENS有明显的即刻镇痛作用。Objective:To explore the analgesia effective of TENS on post-operative pain after total knee replacement.Method: 42 patients were randomly allocated into two groups:TENS and placebo group.The Visual Analogue Scale was used to assess the degree of post-operative knee pain. Repeated Measures ANOVA(SPSS version 10) was used to analyze the VAS data before,among and after TENS or placebo stimulation.Result: A significant difference between-groups in the decrease of VAS scores was found after TENS but not placebo stimulation.The pre-stimulation baseline VAS score of the TENS group gradually dropped from 100% to 83.3%±14.9% at 60 min during,and to 85.7(±16.1)% at 20 min after TENS,with a gradual return to 94.3(±7.8)% at 60 min after TENS.These scores were significantly smaller than those recorded in the placebo group, which showed negligible changes from the baseline value.Conclusion:TENS has a immediate analgesia effect on post-operative knee pain after total knee replacement.
