Classification and Distribution of Extended-spectrum β-Lactamases Produced by Escherichia coli in China: A Meta Analysis of Their Susceptibility

摘  要:目的比较研究全国11个地区大肠埃希菌(E.coli)产超广谱β- 内酰胺酶(ESBLs)及其耐药性分布情况。方法应用荟萃分析法分析全国11个地区2 265株大肠埃希菌产超广谱β- 内酰胺酶及其耐药性。结果全国11个地区E.coli. ESBLs检出率从1.8 33%到45 .00%,呈现从北到南逐步升高的趋势;全国11 个地区E.coliBSBLs的基因型主要为CTX -M型,次为SHV型,但成都主要型为SHV型和TEM型;全国11 个地区产ESBLsE.coli IPM耐药率均低于90 00%, 大部分地区SCF和AMK耐药率<50 .00%, 绝大部分地区FEP和CIP耐药率均>5.0 00%。结论全国11个地区E.coli.检出率均较高, 并呈现从北到南逐步升高的趋势;E.coli所产ESBLs的基因型主要为CTX M型,次为SHV型;产ESBLs E.coli对IPM最敏感,SCF和AMK次之,FEP和CIP基本无效。OBJECTIVE To study the susceptibility, classification and distribution of the extended-spectrum β-lactamases (ESBLs) produced by Escherichia coli in China. METHODS To analyze the ESBLs produced by 2 265 strains of E.coli in 13 published studies from 11 cities of China in recent years with Meta analysis methods. RESULTS The total production rate of ESBLs was from 18.33% to 45.00%, it appeared crescent from north to south. The main genotype of ESBLs was CTX-M, next was SHV, but in Chengdu it was SHV and TEM, respectively. The resistant rate of IPM was lower than 90.00%, the resistant rate to SCF and AMK in a lot of cities was lower than 50.00%, the resistant rates to FEP and CIP was higher than 50.00% in most cities. CONCLUSIONS In 11 cities of China in recent years, the total production rate of ESBLs is CTX-M, next is SHV; IPM is the most susceptible to E. coli producing ESBLs, following SCF and AMK; FEP and CIP are invalid.
