隐动脉皮瓣的临床应用

摘　　要：目的:探讨隐动脉皮瓣在修复下肢软组织损伤中的临床应用。方法:对8例下肢皮肤软组织缺损患者行隐动脉皮瓣顺行或逆行转位修复术,其中2例为膝部损伤,3例为踝关节前方皮肤缺损,2例为足跟部损伤,1例为足底皮肤溃疡恶变。皮瓣最大切取面积12cm×8cm ,最小8cm×5cm ,平均为10cm×7.5cm。结果:8例全部成活,随访6个月至二年不等,皮瓣效果满意。结论:隐动脉皮瓣修复下肢软组织损伤及其他原因所致皮肤缺损,效果可靠,质地满意,不牺牲主要动脉,成功率较高,值得推广应用。Objective:To describe the method of treating soft tissue defects of the legs with the saphenous flaps. Methods:From Jun 2002 to Jun 2004,We had treated eight cases of soft tissue defects with sequent saphenous flaps or reversed saphenous flaps, including two cases in knees, three around the ankles, two of heels and one case with a cancer at the second toe because of protracted inflammation. The sizes of the flaps were from 12 cm×8cm to 8cm×5 cm, average 10 cm×7.5cm; Results:The flaps of all cases had survived, although two cases had partial necrosis in tips. After 6 months to 2 years follow-up, all of the eight cases were evaluated as satisfactory. Conclusion:Treating soft tissue defects of the leg with saphenous flaps is satisfied in evaluations. The flap does not sacrifice the major arteries and the blood supply of the flap is reliable. The method is easy to be performed.
