摘  要:目的探讨反映胰岛素抵抗(IR)的最佳血脂指标。方法引用福田人民医院2003-11体检中无糖尿病史且空腹血糖<5.6mmol/L的人群1002例参加本研究,根据甘油三酯(TG)按百分位数法将研究对象均等地分为3组,以稳态模式法的胰岛素抵抗指数(HOMAIR)评价IR。结果HOMAIR随TG升高而增加,各组间差异有显著性意义(均P<0.01);以HOMAIR为因变量的多元逐步回归分析显示血脂指标中仅TG进入回归方程;TG对IR诊断的敏感性和特异性优于高密度脂蛋白胆固醇、总胆固醇、低密度脂蛋白胆固醇。结论TG是反映IR的最佳血脂指标。Objective To investigate the optimal index of serum lipids to predict insulin resistance(IR) in subjects with normal fasting plasma glucose(FPG).Methods In Nor 2003-11 the 1002 subjects with FPG<5.6mmol/L were selected for study.They were divided into three groups according to tertiles of TG.IR was assessed by the homeostasis model assessment insulin resistance index (HOMA-IR).Results HOMA-IR were increasing with the rising of TG level.There were significant difference in HOMA-IR among three groups (all P<0.05).Multivariate stepwise regression analysis demonstrated that TG remained significantly associated with HOMA-IR.Areas under receiver operating characteristic(ROC) curves showed the specificity and sensitivity of TG in diagnosing IR were significantly higher than that of high-dense lipoprotein cholesterol,total cholesterol,and low-dense lipoprotein cholesterol.Conclusion TG is the optimal index of serum lipids to predict IR in subjects with normal FPG.
