摘  要:目的探讨大肠癌血管新生、嵌合体血管(MV)与临床分期之间的关系及临床意义。方法对1999~2002北京朝阳医院消化科49例大肠癌患者的病变组织切片,用抗CD31单克隆抗体标记血管内皮细胞;用抗CEA单克隆抗体标记结肠癌细胞。经免疫组织化学双标染色检测患者瘤组织“热点”区域的微血管密度(MVD)、MV,并分析其与Dukes临床各期的关系。结果在不同Dukes分期的大肠癌组织标本中,MVD计数分别为B期43.98±21.46,C期59.54±26.95,D期70.80±19.04,表明大肠癌组织MVD随疾病分期的升高而增加,D期明显高于B期(P<0.01)。MV在B期占40.0%;C期占60.0%;D期占87.5%,表明MV随大肠癌的临床分期的增加而增加。经随访发现,随着大肠癌分期、MVD及MV的增加,患者生存率降低,生存期短。表明MVD与大肠癌的预后相关。结论大肠癌MVD及MV与疾病的分期及预后有关,提示临床对高MVD及MV患者应积极治疗。大肠癌血管新生及MV的存在为临床治疗提供了新的靶位点。Objective To investigate angiogenesis and mosaic vessel in colorectal carcinoma.Methods Forty-nine patients suffering from colorectal carcinoma were studied.Endothelial cell was labelled with monoclonal anti-human CD_~31 antibody,tumor cell was labelled with monoclonal anti-human CEA antibody.Endothelial cell and tumor cell were labelled by double-stained immunohistochemistry.The quantity of microvessel was estimated by counting microvessel density (MVD) at hot spots,and the quantity of mosaic vessels (MV) and double-stained cells were estimated at the same visual field.The relationship between mosaic vessels,microvessel density and Dukes staging were analyzed.Results The MVD in colorectal carcinoma 'hot spots' was counted.The MVD of Dukes staging B was 43.98±21.46,staging C was 59.54±26.95,and staging D was 70.80±19.04.MVD increased gradually accompanied by clinical staging.There was statistical difference between staging B and D (P<0.01).It was found that the incidence of MV at staging B was 40%,staging C 60% and staging D 87.50%.There was close relationship between MV and Dukes staging of colorectal carcinoma.It was shown that overall survival decreased with the increase of clinical staging,and that the patients with high MVD and MV had poor prognosis.Conclusion In colorectal carcinoma,patients with high MVD and MV need to be intensively treated.This work provides a new target for the treatment of colorectal carcinoma.
