酚糖脂-I(PGL-I)多克隆抗体在麻风诊断和鉴别诊断中的应用价值

摘　　要：目的探讨采用酚糖脂-I(PGL-I)多克隆抗体检测皮肤活检标本中PGL-I抗原对麻风诊断和鉴别诊断的价值。方法采用PGL-I多克隆抗体的免疫组织化学技术检测50例麻风病、9例其他分支杆菌和真菌病以及10例非感染性皮肤病皮肤活检标本中的PGL-I抗原,并与BCG抗原检测和抗酸染色的结果进行对比分析。结果PGL-I抗原20例多菌型麻风全部阳性,30例少菌型麻风中27例阳性,9例其他分支杆菌、真菌病以及10例非感染性皮肤病均无表达。BCG抗原20例多菌型麻风全部阳性,30例少菌型麻风中28例阳性,9例其他分支杆菌、真菌病中8例阳性,10例非感染性皮肤病未见表达。抗酸染色20例多菌型麻风全部阳性,30例少菌型麻风中18例阳性,9例其他分支杆菌、真菌病中7例阳性,10例非感染性皮肤病全为阴性。结论采用PGL-I多克隆抗体的免疫组织化学技术检测皮肤活检标本中PGL-I抗原,其检测麻风杆菌的特异性显著高于BCG抗原检测和抗酸染色(P<0.01),敏感性显著高于抗酸染色(P<0.01),为麻风病早期诊断和鉴别诊断提供了一种新的方法。Objective To investigate the application value of detection of PGL-I antigen by using PGL-I polyclonal antibody in the diagnosis of leprosy.Methods PGL-I and BCG antigens were detected in 20 multibacillary leprosy cases,30 paucibacillary leprosy cases,9 other mycobacterial or fungal cases, and 10 non-infectious cases,by immunohistochemical staining using PGL-I polyclonal antibody and BCG polyclonal antibody.Acid-fast staining was performed simultaneously.Results PGL-I antigen was positive in all 20 multibacillary leprosy cases,27/30 paucibacillary leprosy cases,negative in all 19 non-leprosy cases.BCG antigen was also positive in all the 20 multibacillary leprosy cases,28/30 paucibacillary leprosy cases,8/9 other mycobacterial or fungal cases, negtive in the 10 non-infectious cases.Acid-fast staining was positive in all the 20 multibacillary leprosy cases,18/30 paucibacillary leprosy cases,7/9 other mycobacterial or fungal cases,negative in all the 10 non-infectious cases.The PGL-I polyclonal antibody method was found to have the highest specificity among the three staining methods studied(P<0.01)and its sensitivity was much higher than that of acid-fast staining(P<0.01).Conclusion The detection of PGL-I in skin biopsy by using its polycolonal antibody proves to be a new specific and effective method for the diagnosis and differential diagnosis of leprosy.
