两种糖脂抗原在结核病血清诊断中的应用

摘　　要：以结核杆菌主要的酚糖酯ＰＧＬＴｂ１和２，３－二酰基海藻糖－２－硫酸酯ＳＬⅣ为抗原，用ＥＬＩＳＡ法检测１２０例肺结核患者、１１２例健康人血清中抗ＰＧＬＴｂ１和ＳＬⅣ的ＩｇＧ和ＩｇＭ抗体，并与ＰＰＤ抗原进行比较。结果表明：ＳＬⅣ－ＩｇＧ－ＥＬＩＳＡ、ＳＬⅣ－ＩｇＭ－ＥＬＩＳＡ、ＰＧＬＴｂ１－ＩｇＧ－ＥＬＩＳＡ、ＰＧＬＴｂ１－ＩｇＭ－ＥＬＩＳＡ和ＰＰＤ－ＩｇＧ－ＥＬＩＳＡ的特异性分别为９６．４３、９６．４３、９６．４３、９６．４３和９５．５３％；敏感性分别为５１．６７、３２．５０、１４．１７、１８．３３和３３．３３％，符合率分别为７３．２８、６３．３６、５３．８８、５６．０３和６２．９３％，阳性预测值分别为９６．８７、８６．６７、８０．９５、８４．６２和８８．６４％。３种抗原中以ＳＬⅣ最好。Sera from 112 healthy controls and 120 pulmonary tuberculous patients(61 untreated patients and 59 active patients)were assayed, by ELISA, to test the activity of IgG and IgM antibodies against antigen of 2,3-diacyl-trehalose-2'-sulphate(SL Ⅳ)a phenolglycolipid antigen(PGLTb1)and purified protein derivative(PPD)from M. tuberculosis.Respectively,for SLⅣ-IgG-ELISA,SLⅣ-IgM-ELISA,PGLTb1-IgG-ELISA,PGLTb1-IgM-ELISA, PPD-IgG-ELISA,the specificities were of 96. 43, 96. 43, 96. 43, 96. 43 and 95. 53%;the sensitivities were of 51. 67, 32. 50, 14. 17, 18. 33 and 33. 33%;the efficiencies were of 73.28, 63. 36, 53. 88, 56. 03 and 62. 93%; the predictive values for a positive result were of 96.87, 86. 67, 80. 95, 84. 62 and 88. 64%.Among the three antigens tested,SLⅣ was found to be better than PGLTb1 and PPD.
