不同剂量辛伐他汀对颈动脉粥样斑块的临床干预

摘　　要：目的:观察不同剂量辛伐他汀对颈动脉粥样斑块的临床干预。方法:174例高胆固醇血症病人随机分为辛伐他汀10mg及20mg治疗组和对照组,分别测定随访6mo前后的总胆固醇(TC),低密度脂蛋白胆固醇(LDLC),颈动脉粥样斑块的Crouse测定法积分(斑块积分)和颈总动脉的内膜中层厚度(IMT)。结果:辛伐他汀20mg治疗组TC,LDLC降低了(2.4±s0.8),(2.1±0.8)mmol·L-1,10mg治疗组TC,LDLC降低了(1.8±0.6),(1.0±0.4)mmol·L-1,2个治疗组均有明显的降TC和LDLC作用(均P<0.01),20mg治疗组降TC与LDLC较10mg治疗组疗效更明显(P<0.01)。10mg治疗组与对照组治疗前后斑块积分和IMT比较均无明显差异(P>0.05),而20mg治疗组则治疗前后斑块积分和IMT明显减少(P<0.05)。结论:辛伐他汀2种剂量均可显著降低TC和LDLC水平,且辛伐他汀20mg对颈动脉粥样斑块有消退作用。AIM: To investigate the clinical beneficial effects of simvastatin on carotid atheros clerotic plaques in different doses. METHODS: One hundred and seventy-four patients with hypercholesterolemia were randomly a s signed to simvastatin 10 mg·d^(-1), simvastatin 20 mg·d^(-1) and control groups. The levels of total cholesterol( TC), low-density lipoprotein cholesterol(LDL-C), carotid plaques and intima-m edi a thickness(IMT) were measured before and after 6 mo treatment. RESULTS: The reduced levels of TC and LDL-C in simvastatin 20 mg group were ( 2.4±(0.8) mmol·L^(-1)),(2.1±(0.8)) mmol·L^(-1), in simvastatin 10 mg group were(1.8 ±0.6) mmol·L^(-1),((1.0±)(0.4)) mmol·L^(-1). The two groups with two dif ferent doses simvastatin reduced TC and LDL-C significantly(P<0.01). Ther e were significant differences in LDL-C and TC levels existing between simvasta tin (20 mg) group with simvastatin 10 mg group (P<(0.01)). Simvastatin 20 mg g roup had a significant effect on decreasing carotid plaques and IMT(P<(0.0 5) ). There were no distinct changes shown about carotid plaques i n simvastatin 10 mg group and the control group(P>0.05). CONCLUSION: Treatment with two different doses of simvastatin can reduce the levels of LDL- C and TC, but simvastatin 20 mg can further resolve the carotid atherosclerotic plaques.
