摘  要:作者运用兔跟腱体外培养的实验模型,证实了在跟腱愈合过程中,腱围组织起着关键性的桥接作用。虽然在横断的肌腱部分,腱细胞也有试图修复的表现,但如没有腱围组织的参与,跟腱的愈合将可能是不完全的。根据实验结果,作者提出在进行跟腱修复手术时,一定要尽可能保留腱围组织以利于跟腱的愈合。One hundred and twenty-four of segments of Achilles tendon were obtained from 16 New Zealand rabbits. A“V”cut was made in the middle portion of each individual segment.After culture for 3 or 6 weeks under normal or low(2%)oxygen condition,a combination of regenerative and degenerative changes have been found.Among them,the predominant change was in paratenon,it proliferated,became much thicker and potentially started bridging the “V”cut area especially in normal oxygen group. Besides, the cells inside tendon migrated to the media and formed a confluent fibroblast-like layer on the bottom of petri-dishs in 3 weeks groups. Cell proliferation in paratenon was also identified by autoradiography with H3-thymidine labelling.Collagen typing revealed decrease of collagen type I inside the tendon and increase of collagen type Ⅲ along the margin of cutting area.The results corresponded to previous reports in regard of the intrinsic healing potential of tendon,though, paratenon played the most important role in the whole process of healing.The authors suggest that during the operation of repairing Achilles tendon, the paratenon should be reserved.If it is difficult to suture it back again, then a graft of paratenon from non-injury area is needed in order to cover the anastomotic portion of the tendon.
