耻骨结节处银质针松解术治疗股内侧、膝关节内侧疼痛的疗效评估

摘　　要：目的:观察银质针松解术治疗对大腿内侧、膝关节内侧疼痛的改善效果。方法:选择2001-08/2004-08解放军总医院康复医学科门诊及病房诊治无外伤的大腿内侧、膝关节内侧疼痛患者75例,作为耻骨结节肌肉附着处软组织慢性损伤治疗组,采用一次耻骨结节银质针松解术进行治疗,治疗后3,6个月进行2次随访。以20例同类患者为对照组,不进行特殊治疗,随访3个月。治疗组与对照组在性别、年龄、病程上均具有可比性,同时对患侧耻骨结节压痛点采用目测类比评分法评估疼痛改善情况(10分为严重,0分为无痛),于治疗前,治疗后3,6个月进行评分,并对患者进行自觉症状改善情况评分。结果:按实际处理分析,治疗组治疗后3个月随访67例,6个月随访54例,对照组3个月随访20例。①治疗前治疗组与对照组目测类比评分接近(6.4±0.7,6.8±0.6,t=77.1,P>0.05),治疗组3个月后目测类比评分较治疗前明显减低(3.3±0.3,6.4±0.7,t=55.9,P<0.001),与对照组比较明显减低(3.3±0.3,6.6±0.7,t=69.3,P<0.001)。②治疗组症状改善评分较对照组明显提高犤(46.7±0.87)%,(0.25±0.03)%,t=66.4,P<0.001犦。③6个月随访时治疗组目测类比评分与治疗前相比明显减低(3.8±0.4,6.2±0.6,t=48.4,P<0.001),症状改善评分与治疗前比明显提高(t=51.1,P<0.001)。结论:银质针松解术治疗能解除肌痉挛和肌挛缩,消除压痛点,改善患者疼痛和不适的自觉症状,效果确切。AIM:To observe the effect of release using silver needle acupuncture in impro ving femoral medial and knee joint medial pain. METHODS:Seventy- five nontraumatic patients with femoral medial and knee joi nt medial pain were treated in the outpatient department or wards of Department of Rehabilitation Medicine,General Hospital of Chinese PLA between August 2001 a nd August 2004.Their chronic injury of soft tissues attaching to pubic tubercle were treated with release using once silver needle acupuncture on pubic tubercle as the treatment group,and followed up at postoperative 3 and 6 months.Another 20 patients with the same diseases as above were selected as the controls,but th ey were not treated specially,and followed up at postoperative 3 months.The pati ents between the treatment and control groups were comparable in their genders,a ges and illness courses.The tenderness point on pubic tubercle of the affected s ides were scored by visual analogue scale(VAS) to assess the symptom improvement (10 points were severe and 0 was painless) before treatment,at 3 and 6 months af ter treatment,and meanwhile,the improvement of subjective symptom was scored in the patients. RESULTS:According to the intention management,67 patients were followed up at 3 months,54 patients at 6 months in treatment group,and 20 at 3 months in the c ontrol group.① Before treatment,the VAS scores were similar in the treatment an d control group,6.4± 0.7 and 6.8± 0.6 respectively(t=77.1,P >0.05),but at 3 mo ths those in the treatment group(3.3± 0.3) were significantly decreased as comp ared with those before treatment(t=55.9,P< 0.001),and also significantly decreas ed as compared with those in the control group(6.6± 0.70)(t=69.3,P< 0.001).② The improvement rate of symptom in the treatment group was significantly higher than that in the control group[(46.7± 0.87)% vs (0.25± 0.03)% ,t =66. 4,P< 0.001].③ The VAS scores in the treatment group was significantly decreased at 6 months after treatment,as compared with those before treatment
