新鲜羊膜与自体角膜缘上皮移植对急性眼化学烧伤的修复

摘　　要：目的:观察新鲜羊膜与自体角膜缘上皮移植对急性眼化学烧伤的修复作用,为临床急性眼化学烧伤的视功能恢复探索更为有效的方法。方法:2001-01/2002-08南方医科大学珠江医院眼科收治急性眼化学烧伤患者12例12眼,于伤后1～5d行自体角膜缘上皮移植联合新鲜人羊膜移植重建眼表,随访观察羊膜植片情况、眼表上皮化时间和视力恢复情况。结果:随访6～12个月,12例12眼眼表面重建成功,未见新鲜羊膜植片急性排斥反应,及感染发生;无新生血管、假性胬肉及角膜上皮缺损。7眼角膜透明,3眼角膜云翳,1眼角膜斑翳,1眼角膜白斑,视力均不同程度的提高,11眼脱盲。结论:新鲜人羊膜移植联合自体角膜缘上皮移植是修复急性眼化学烧伤的一种有效方法。AIM:To evaluate the restoration of acute ocular chemical burns by fresh human amniotic membrane transplantation(AMT) and limbal epithelial autograft so as to find out effective clinical methods for the recovery of visual function in acut ely burnt eyes. METHODS:Twelve eyes in 12 patients suffering from acute ocular chemical burns were selected from the inpatients in the Department of Ophthalmology,Zhujiang H ospital,Southern Medical University from January 2001 to August 202.They were pe rformed by limbal epithelial autograft and fresh human amniotic membrane to reco nstruct the ocular surfaces from 1 to 5 days after burn.All patients were follow ed to observe the amniotic membrane graft,the time for cornea epithelization and the restoration of visual function. RESULTS:After follow- up for 6 to 12 months,the ocular surfaces in all 12 ca ses(12 eyes) were reconstructed,and no acute rejection and infection of amniotic membrane were found.Neovascularization,pseudopterygium invasion and corneal epi thelium defect were also not foumd.Corneal transparency was found in 7 eyes,corn eal nebula in 3 eyes,corneal macula in 1 eye,corneal leucoma in 1 eye.Visions in all the cases were improved to different extents,of which 11 had escaped blindn ess. CONCLUSION:Fresh human AMT associated with limbal epithelial autograft is an effective restoration for acute chemical burns of eyes.
