摘  要:目的:探讨静脉给予L-703,606后严重烫伤大鼠P物质免疫活性含量的变化规律。方法:实验于2003-03/2004-01在第四军医大学西京医院烧伤外科实验室完成。选择SD大鼠76只,按随机数分为正常对照组(4只),烫伤对照组(24只),L-703,606组(24只),β-七叶皂甙钠组(24只),运用放射免疫测定方法,动态检测经尾静脉给予神经激肽1受体非肽类拮抗剂L-703,606(250nmol/kg)后深II度烫伤大鼠(20%TBSA)烫伤后早期72h创周及空肠P物质免疫活性物质含量的变化。结果:76只大鼠进入结果分析。①与正常对照组相比,L-703,606组、β-七叶皂甙钠组大鼠创周及空肠P物质免疫活性物质含量同烫伤对照组具有类似规律。烫伤后早期(1h)P物质免疫活性物质含量明显升高,(P<0.01),但均低于烫伤对照组(P<0.01);而后逐渐下降。与烫伤对照组相比,L-703,606组P物质免疫活性物质含量的病理性改变明显较轻,且轻于β-七叶皂甙钠组。结论:神经激肽1受体非肽类拮抗剂L-703,606能抑制严重烫伤大鼠早期P物质免疫活性物质含量的变化,为减轻严重烧伤后早期包括血管通透性增加、血浆蛋白外渗等炎症反应及局部和全身水肿形成提供新的思路。AIM:To investigate the change regularity of content of immunoreactive substan ce P(iSP) in rats after deep partial- thickness skin scald in early stage given with nonpeptide neurokinin 1(NK1)receptor antagonist L- 703,606 intravenously. METHODS:The experiment was conducted in the laboratory for Department of Burn Surgery,Xijing Hospital,Fourth Military Medical University from March 2003 to J anuary 2004.Seventy- six SD rats were divided into normal control group(n=4),sc ald control group(n=24),L- 703,606 group and sodium aescinate group(n=24) accor ding to their random number.Radioimmunoassay was used to dynamically observe and detect the content changes of iSP beside the wound and in the jejuna of rats in early stage(72 hours) of deep partial- thickness skin scalding(20% TBSA) aft er given with nonpeptide NK1- receptor antagonist L- 703,606 intravenously(250 nmol/kg) through tail vein injection. RESULTS:All the 76 rats were included in the result analysis.As compared with the normal control group,there was similar regularity in the content changes of iSP beside the wound and in the jejuna of rats in the L- 703,606 group and sod ium aescinate group.The content of iSP was significantly increased in the early time(1 hour) after burns(P< 0.01),but lower than that in the scald control group (P< 0.01);then increasingly descended.As compared with the scald control group,t he pathological changes of iSP content in the L- 703,606 group were alleviated and also less than those in the sodium aescinate group. CONCLUSION:Nonpeptide NK1- receptor antagonist L- 703,606 given intravenous ly can partially inhibit the content changes of iSP in rats after deep partial- thickness skin scald in early stage,which may lead to a new path to relieve the inflammatory reaction such as increase of vascular permeability and plasma prot ein extravasation,and formation of local or general oedema after severe burn.
