中心静脉置管感染患者C反应蛋白、炎症细胞因子的表达及其意义

摘　　要：目的 :探讨中心静脉置管感染患者 C反应蛋白 (CRP)、及炎症细胞因子、血清肿瘤坏死因子 (TNF- α)和白介素 - 6 (IL- 6 )表达的变化。方法 :对血液透析行中心静脉置管发生感染患者39例进行临床分析及 CRP、TNF- α、IL- 6的表达进行检测 ,同期血液透析患者 30例为对照组。结果 :感染组患者 TNF-α、IL - 6表达均高于对照组 ,有显著差异 (P<0 .0 1) ,C反应蛋白阳性率感染组也较对照组明显增高 ,仅导管口皮下感染者 TNF- α、IL- 6表达低于发生败血症组 (P<0 .0 5 )。结论 :监测 CRP、TNF-α、IL - 6的表达对中心静脉置管感染及发生导管败血症有一定的早期监测作用。Objective:To investigate the alt erations of the expressions of C-re active protein(CRP),inflammatory cell factor:serum tumor-necrotizing factor(TNF - α)and interleukin-6(IL-6) in patients with intravenous catheter-induced infe cti on (ICII). Methods:Clinical analysis and dete ction of the expressions of CRP, T NF-α and IL-6 in 39 patients with infection induced by intravenous catheter w ho received hemodialysis and in 30 patients undergoing this therapy at the same ti me as a control group,were performed. Results:T he expressions of TNF-α and IL- 6 in infection group were higher than those in control group,with significant di fference(P<0.01).CRP positive rate in infection group was markedly higher tha n that in control group.However, TNF-α and IL-6 expressions in patients with su bcutaneous infection at the site of catheterization were lower than those in pat ients with septicemia(P<0.05).Conclusion:Det ecting the expressions of CRP, T NF-α and IL-6 has the early monitoring effect on the occurrence of ICII and c atheter-induced septicemia.
