重症肺炎患儿血中促胃液素、胃动素及生长抑素水平变化及意义

摘　　要：目的了解胃肠激素在重症肺炎发病过程中的作用及临床意义。方法对重症肺炎组、普通肺炎组、正常对照组各40例,应用放射免疫法测定其血中促胃液素(GAS)、胃动素(MTL)及生长抑素(SS)水平,同时测定其动脉血氧分压(PaO2)。重症肺炎组有28例治愈或好转出院时复查了GAS、MTL及SS。结果重症肺炎患儿血中GAS、MTL及SS水平较普通肺炎组和对照组均明显增高(P<0.01);普通肺炎组GAS较对照组高(P<0.05),而MTL和SS水平与对照组比较差异无显著性(P>0.05);重症肺炎患儿治疗后GAS、MTL和SS均明显低于治疗前的水平(P分别<0.01、<0.01、<0.05);重症肺炎患儿PaO2明显低于正常对照组(P<0.01),且与血中的GAS、MTL、SS水平均呈较好的负相关(r分别为-0.611、-0.35及-0.838,P分别<0.01、<0.05及<0.01)。结论重症肺炎患儿血中GAS的增高可能是其易并发消化道出血的原因之一;支气管肺炎患儿血中GAS、MTL、SS水平可作为病情轻重的监测指标。Objective To explore the changes and clinical significance of gastrin(GAS),motilin(MTL) and somatostatin(SS) in the children with severe pneumonia. Methods There were 80 children with bronchopneumonia. They were distributed into two groups according to clinical pattern: mild(n=40) or severe pneumonia group(n=40). The control group consisted of 40 health children. All serum or plasma levels of GAS,MTL and SS were measured by radioimmunoassay. Twenty-eight cases in severe pneumonia group were re-measured when they were recovered. Results The serum or plasma levels of GAS,MTL and SS were significant higher in severe pneumonia group than those in mild pneumonia group (all P<0.01) and control group (P<0.01),respectively. Compare with controls, the level of GAS was significantly higher in mild group(P<0.05), however, there were no statistically significant difference in serum levels of MTL and SS(P>0.05). Significant decrease of serum levels of GAS,MTL and SS were observed in severe pneumonia group(P<0.01, P<0.01, P<0.05,respectively) after therapy.Conclusion Increasing of serum level of GAS may be one of cause that result in gastrointestinal bleeding easily in the children with severe pnuemonia. The serum or plasma levels of GAS,MTL and SS may be acted as new markers which evaluate the severity of the children with bronchopneumonia.
