舒适护理对消化道手术病人留置胃管时机选择的临床研究

摘　　要：[目的]对两种不同时机留置胃管病人的舒适度进行研究,以指导临床护理,推行舒适护理。[方法]对164例消化道手术病人进行术前舒适需求调查,观察不同时机留置胃管时病人血压、心率的变化以及恶心呕吐、误入气管发生率以及舒适程度的比较。[结果] 93 .9%的病人希望在全身麻醉下留置胃管;对照组病人留置胃管时、留置胃管后血压、心率有较大波动,而实验组的波动则不明显,而且两组病人的血压、心率比较有统计学意义(P <0 .0 1或P <0 .0 5 )。两组病人置管一次成功率及所用时间比较无统计学意义(P >0 .0 5 )。两组病人恶心呕吐、误入气管发生率比较有统计学意义(P <0 .0 1或P <0 .0 5 )。舒适度比较有统计学意义(P <0 .0 1)。[结论]全身麻醉下留置胃管符合病人的舒适需求,并且极大地提高了消化道手术病人置管舒适度。Objective:To investigate on comfort degree in patients underwent indwelling gastric tube at two different stages, in order to guide clinical nursing and carry out comfort nursing. Method:A total of 164 patients underwent digestive tract operation were investigated on demand for comfort degree before operation. Then to observe the changes of blood pressure, heart rate of patients underwent indwelling gastric tube at different stages and compared the occurrence of nausea, vomiting, trachea aspiration of them. Result:Among them, 93.9% patients hoped to have gastric tube indwelled under general anesthesia. The blood pressure and heart rate fluctuated heavily during indwelling and after the gastric tube indwelled in control group patients. While these changes could not be observed obviously in test group. There was a statistical difference in blood pressure and heart rate and the occurrence of nausea(P<0.01 or P<0.05), vomiting and airway aspiration of patients between the two groups(P<0.01 or ). There was no statistical difference in one-time success rate and the time spending on indwelling tube between the two groups of patients(P>0.05). Postoperative follow-up visit showed that there was a statistical difference in terms of comfort degree of patients between the two groups(P<0.01).Conclusion:It suggested that indwelling gastric tube under general anesthesia could meet patients' demands for comfort, which can enhance the comfort of indwelling gastric tube for patients undergoing digestive tract surgery greatly.
